TITLE OF RFQ: Pest Control for City of Waukee Facilities
The City of Waukee is seeking a service provider (Contractor) to provide Pest Control
Services at the following locations:

A. DESCRIPTION OF WORK AND SCOPE OF SERVICES. Contractor shall furnish
complete Pest Control Services, as defined in Attachment 1 – Pest Control Contract
Specifications.
B. LOCATIONS AND FREQUENCY OF SERVICE. Services to be provided on a monthly
basis as outlined at the locations listed in Attachment 1 – Site List/Bid Form.
C. TERM OF CONTRACT
Initial Contract Begin/End Dates: January 1, 2027 – June 30, 2027
Initial term of the contract: 6 months
Number of possible contract extensions: 3 additional 1-year terms (July 1, 2027 – June
30, 2030)
D. CONTRACTUAL TERMS AND CONDITIONS.
1. INSURANCE. $1,000,000 General Liability minimum coverage is required of the
Contractor upon award of the Contract.
2. EVALUATION AND SELECTION. Bids will be evaluated and the contract for all
locations to the responsible Contractor submitting the lowest total Bid. The
selection will be subject to the final approval of the City.
3. FORM AND CONTENT OF BIDS. Failure to adhere to the Bid format may result in
rejection of the Bid. Bid Packets shall include the following:
The Contractor shall provide the following general information
a. Name, address, telephone number, fax number and e-mail address of the
Contractor including all d/b/a’s or assumed names or other operating names
of the Contractor and any local addresses and phone numbers.
b. Name, address and telephone number of any personnel (i.e. Account Rep)
who will be assigned to Agency’s account if Contractor is awarded the
contract for these services.
c. Completed Attachment 1 – Site List/Bid Form Contractor’s Bid shall
include an all-inclusive, total cost in U.S. dollars (including all travel,
expenses, etc.) to provide the requested services. All pricing to be based on
Net 60 Days Payment Terms. All fees expected for payment are to be
included in this Bid. The City of Waukee is a tax exempt organization. A tax
exempt certificate will be provided to the bidder awarded this contract.

Attachment 1 – Site List/Bid Form.

4. Bids may be delivered electronically (via email) or hard copy (mail or hand
delivery).
a. For Bids submitted electronically: attach all bid documents to an email
addressed to City of Waukee Facility Manager Christine Maduro at the
email and physical address list in this RFQ. Subject line of the email shall be:
“RFQ City of Waukee Pest Control Services – Contractor’s Name”.
b. For Bids submitted via hard copy: place all bid documents in a sealed
envelope labeled with the following information:

c. The City must receive the Bid at the Issuing Officer’s address prior to the
date/time listed on the RFQ Schedule. Late bids will not be accepted. It is the
Contractor’s responsibility to ensure that the Bid is received prior to the
deadline. The City shall not be responsible for misdirected packages or
technical issues (for electronically submitted Bids).
E. MISCELLANEOUS
1. The RFQ document and any addenda will be posted at
https://www.waukee.org/Bids.aspx.
2. Contractors are invited to submit written questions and requests for clarifications
regarding the RFQ to the Christine Maduro (cmaduro@waukee.org). The questions
or requests for clarifications must be via email.
3. Sites will be available for exterior inspections only the week of October 19, 2026.
Please schedule your visits with Waukee Facilities Manager Christine Maduro
(cmaduro@waukee.org). Interior inspections will not be available prior to the bid
deadline listed on the RFQ.
4. The costs of preparation and delivery of the Bid(s) are solely the responsibility of
the Contractor.
5. All Bids become the property of the City and shall not be returned to the
Contractor.
6. Notice of Intent to Award the Contract(s) will be sent to all Vendors submitting a
timely Bid and will be posted at the website shown on the RFQ cover sheet
7. The City does not guarantee any minimum level of purchases under the Contract.
ATTACHMENT 1 – Pest Control Contract Specifications
A. General Requirements
Contractor will provide pest control services in accordance with Integrated Pest
Management principles. Mechanical, physical, and cultural controls, as well as
surveillance practices for pests will be used in addition to chemical controls, to
provide the safest, most effective, least environmentally damaging control method.
Contractor must have a Commercial Pesticide Applicator Company License with
the State of Iowa, Department of Agriculture and Land Stewardship (IDALS), and
provide proof of that license prior to contract award.
Contractor must provide a Pesticide Use Proposal (PUP) list of all chemicals which
may be used to control pests on City of Waukee property to the Waukee facilities
manager. That list must include, at a minimum, the commercial name of the
pesticide, the EPA registration number of that pesticide, and the target pests. The
PUP will be provided prior to award of the contract will be updated at least annually
(by July 1). Product labels and material safety data sheets shall be available for each
product.
B. Police Station – Special Pet/K9 Safety Requirements
Pest control services shall be performed using a pet/K9-sensitive protocol. For
rodent control, mechanical trapping and other non-chemical control methods shall
be utilized whenever practical. When rodenticide is necessary, bromethalin-based
products shall not be used, and preference shall be given to anticoagulant
rodenticides such as bromadiolone. All rodenticide must be contained within
secured, tamper-resistant bait stations designed to resist access by dogs and
located in areas inaccessible to animals. No loose rodenticide bait shall be used in
areas where dogs may be present. All other pesticide applications shall be made
according to product labeling and in a manner that minimizes potential exposure to
pets and working dogs.
C. Licensing Requirements
All Contractor personnel who apply pesticides shall have a current Commercial
Pesticide Applicator Certificate in the appropriate category, as issued by IDALS. All
pesticides must be applied in strict accordance with the manufacturer’s label.
Equipment used by the Contractor shall be in good condition. Pesticides carried on
Contractor vehicles shall be locked securely when the applicator is not present. No
pesticides or Contractor equipment can be stored on City property between visits.
Contractor must have appropriate safety and spill response equipment when
applying pesticides. Contractor must carry valid identification and communicate
and coordinate with the Waukee facilities manager prior to the pest management
service calls. Personnel assigned to provide services at the Waukee Public Safety
Building must provide a clear background check from the Department of Criminal
Investigation in Des Moines, Iowa. Clear background checks must be filed with the
Waukee Police Department prior to contract start date.
D. Reporting Requirements
For each facility visit, the Contractor shall provide the following information
electronically, on the invoice or separately: Name and location of facility services,
date and time of visit, results of any surveillance for pests, and the name and
applicator certificate number for anyone applying pesticides. If pesticides are
applied, the commercial name, EPA product number, concentration, dilution,
amount applied, and target pest must be included for each pesticide application.
General comments about the condition of the facility in relation to pest
management should be included and photos of findings as applicable.
E. Service Frequency and Scope
Furnish complete pest control service on a monthly basis to control rodents,
cockroaches, ants, centipedes, silverfish, earwigs, common beetles, boxelder bugs,
stink bugs, Asian beetles and other arthropods present inside the buildings.
Callbacks for specific issues will be at no extra charge to the owner.
