PERFORMANCE WORK STATEMENT

for

MARINE CORPS LOGISTICS BASE (MCLB) ALBANY SOLID STATE MEDIA DISINTEGRATOR

(SSMD) EXTENDED WARRANTY AND PREVENTATIVE MAINTENANCE SERVICE





1.0 Introduction: MCLB Albany requires an Extended Warranty and a Preventive Maintenance service contract for

one (1) Data Security, Inc. (DSI) SSMD-2mm SSMD. This service is essential to maintain the operational readiness

and efficiency of the equipment. The contract will provide a maintenance service and warranty for twelve months

period which includes parts and labor and onsite servicing.





2.0 Scope: The purpose of this service contract is to provide comprehensive support for the DSI SSMD-2mm. This

service encompasses both a one-time on-site preventive maintenance visit and a one-year extended warranty.





3.0 Hours of Operation: All work will be conducted during the normal operating hours of 8:00 AM through 4:30

PM Eastern Standard Time (EST), Monday through Friday, excluding federal holidays, aboard MCLB Albany.

5.0 Preventive Maintenance Service: The Contractor shall provide will provide one appointment to inspect, clean,

and perform necessary maintenance on the SSMD-2mm Disintegrator. Scheduling the on-site PM visit shall be

coordinated with Technical Representative 4-6 weeks upon receipt of order for scheduling and performance of

Preventative Maintenance services. The Preventive Maintenance service shall include, but is not limited to, the

following:

a. Inspection of Controls and Safety features.

b. General Cleaning of the equipment.

c. Inspection of the Grinder Stage and parts replacement.*

d. Inspection of the Shredder Stage and parts replacement.*

e. Inspection and replacement of the Air filter.*

f. Visual inspection of the Motor and Gearbox.

g. Providing a warranty repair action plan.

h. Providing pricing for non-warranty items and consumable items needing replacement.

i. Providing suggestions for optimal operation.

*Government will provide parts.





5.1.1 Repair Parts: If consumable parts need to be replaced during Preventative Maintenance, these parts will be

purchased in advance by the Government. Parts purchased in advance will be replaced by Contractor during the

Preventative Maintenance.

A common consumable item price list will be provided to the Government to procure parts in advance of scheduled

maintenance. These items include, but are not limited to: belts, chains, liners, cutters, screens, filters, or adhesive

backed foams.





5.2 Extended Warranty Service: The Contractor shall provide an Extended Warranty to repair manufacturer

defects. This warranty shall include parts and labor. The warranty does not cover wear or consumable items.

a. All warranty claims must be reported to the Contractor prior to the end of the Extended Warranty Period.

b. The Contractor shall provide a repair plan within 48 hours on a covered warranty claim.

c. Repairs will be performed on-site at place of performance (as defined in Section 3.1). The Contractor may

also provide an exchange of a product of equal value.

d. Shipping costs are included, excluding expedited shipping. All unserviceable parts that are replaced shall

become the property of the Contractor.

e. Degausser magnetic field verification or certification is not included under this warranty and must be

coordinated separately.





6.0 Contractors Unauthorized Work Performance: The Contractor shall not perform work that deviates from

contract requirements and specifications. If the Contractor deviates from contract requirements and specifications

without approval of the CONTRACTING OFFICER, such deviation shall be at the risk of the Contractor and any

cost related thereto shall be borne by the Contractor.





7.0 Base Access MCAS Albany: Contractors, and/or delivery drivers, must have in their possession: the contract

number, sponsor’s contact information, and delivery/service address to gain initial access to the base.

Contractors must be registered in the Defense Biometrics Identification System (DBIDS). It is the Contractors

responsibility to ensure all their employees and/or subcontractors are properly registered within DBIDS.

DBIDS credentials will be valid for the duration of your contract, not to exceed three years; however, re-vetting of

individuals' information will occur each year.



Contractors requiring access for over 60 days will receive a credential valid for up to one year.

Contractors requiring access for less than 60 days will receive a temporary paper pass.

Contractor Common Access Card (CAC) Eligibility: Contractor personnel authorized CACs are exempt from

DBIDS enrollment.

If you have questions regarding base access, Contractor CACs, or DBIDS requirements, contact the Visitor

Control Center (VCC).

Contractors will report to the Visitor Control Center to confirm their access requirements, undergo a background

check and receive their credential/pass.

Location: The Visitor Control Center is located on Fleming Road at Building 3010, Room 33.

Hours of Operation & Appointments

The Visitor Control Center hours of operation: Monday through Friday from 7:30 a.m. to 3:30 p.m. for

vehicle registration, 30-day passes, and visitor passes. Wednesday and Friday 7:30 a.m. to 3:30 p.m. for

DEERS ID Card Services. The Visitor Control Center is closed for lunch from 11:45 a.m. to 12:30 p.m.

Appointments are required to receive services at the Visitor Control Center. If you would like to make an
