California Children's Services Physical and Occupational Therapy Services

This Request for Proposal (RFP) announces the intent of the County of Mendocino Public Health Department to provide physical and occupational therapy services. This includes the specifications, terms, and conditions for the requested services.

The Mendocino County Public Health (MCPH) Department is seeking a qualified Community-Based Organization (CBO) or private organization in the areas of physical and occupational therapy services. It is the intent of MCPH to award a three year contract (with the option of a one year renewals) to the entity whose response conforms to the RFP and meets the County’s requirements.

The purpose of the RFP is to seek a qualified Community-Based Organization (CBO) to expand the California Children’s Services (CCS) program in the areas of physical and occupational therapy services within the County. This request is extended to any Mendocino County CBO’s and private organizations that can successfully provide services.

Vendors must submit six (6) copies of their proposal:  Five (5) complete paper copies with original Vendor signature, and one (1) complete copy on USB Flash Drive.   The proposal must be formatted in accordance with the instructions of this RFP.  Promotional materials may be attached, but are not necessary and will not be considered as meeting any of the requirements of this RFP.  Proposals must be enclosed in a sealed envelope or package, clearly marked “Mendocino County RFP No. 021-25”, and delivered by 5:00 p.m., October, 13, 2025 to:

Late or facsimile proposals will not be accepted.  It is the proposer’s responsibility to assure that its proposal is delivered and received at the location specified herein, on or before the date and hour set.  Proposals received after the date and time specified will not be considered.  Note:  The unauthorized use of the County’s official logo is strictly prohibited.
