1. Purpose

The Office of Human Services is issuing this Request for Information (RFI) to solicit

information from qualified providers regarding Block Grant funding, specifically the Homeless

Assistance Program (HAP) and Human Services Development Fund (HSDF) funds. This RFI is

for planning purposes only and does not constitute a solicitation for bids, proposals, or quotes.

2. Background

The County is exploring options to enhance access to homeless assistance services, rental

assistance, case management, bridge housing and housing for underserved populations. We are

seeking innovative approaches and capabilities from providers who can deliver evidence-based

and community-centered services.

3. Requested Information

Respondents are requested to provide the following information:

• Organization name, address, and primary contact.

• Overview of services currently provided relevant to this RFI.

• Geographic areas served.

• Description of staffing structure and qualifications.

• Data demonstrating program outcomes, effectiveness, or reach (if available).

• Partnerships or collaborations with other agencies.

• Capacity for scaling services if needed.

• Estimated costs or funding requirements (budget breakdown MUST be included).

4. Response Format

Please limit your response to five (5) pages. Responses may be submitted electronically in PDF

format to Megan Stone at megan.stone.ohs@luzernecounty.org and Kristina Jordan at

k.jordan@luzernecounty.org no later than Friday, August 28, 2026 at 12:00 pm. Please

reference the RFI reference ID and RFI Title in the subject line of your email. Hard copy

submissions will not be accepted.
