1. PUBLIC NOTICE
Crystal Lake Support, Inc. (“Owner”) is soliciting Statements of Qualifications from qualified
professional engineering firms to provide dam inspection, evaluation, design, permitting, and
construction-phase engineering services associated with the repair and rehabilitation of an
existing dam structure impacted by storm-related damage.
This RFQ is issued in accordance with applicable North Carolina procurement statutes, including
N.C.G.S. §143-64.31, and applicable federal requirements if federal disaster or mitigation
funding is utilized, including 2 CFR Part 200.
The Owner intends to select the most qualified firm and negotiate a fair and reasonable fee using
a qualifications-based selection process.

4. RFQ SUBMISSION REQUIREMENTS
Submittals must be received by the deadline listed above. Late submissions will not be
considered.

5. PROJECT BACKGROUND
Crystal Lake Support, Inc. owns and is responsible for the Crystal Lake Dam and associated
spillway structures located in Moore County, North Carolina, within the Lakeview, NC
community. The dam and spillways are integral to the Crystal Lake impoundment and provide
stormwater conveyance, storage, and downstream flow control functions for the surrounding
watershed. A DOT-owned road crosses over a spillway; this road will be initially repaired by
Crystal Lake on a temporary basis under a separate contract, then permanently repaired by NC-
DOT after the dam has been permanently repaired.

The Crystal Lake Dam and spillways sustained damage during the 2025 storm Chantal, resulting
in structural, hydraulic, and erosion-related concerns that require professional dam engineering
evaluation and permanent repairs. A comprehensive dam repair and rehabilitation engineering
services firm is required to repair and restore the facility to safe operating condition and ensure
compliance with North Carolina Dam Safety regulations and any applicable federal
requirements.
Crystal Lake Support, Inc. is seeking a qualified dam engineering firm with demonstrated
experience in dam inspection, hydrologic and hydraulic analysis, geotechnical evaluation,
spillway design, regulatory coordination, and construction-phase engineering support for dam
repair projects of similar scale and complexity.
6. GENERAL SCOPE OF SERVICES
The selected firm will provide professional dam engineering services, which may include but are
not limited to:
• Review of existing dam safety inspection and condition assessment
• Hydrologic and hydraulic analysis
• Geotechnical investigations and stability analysis
• Spillway evaluation and design
• Structural analysis and design
• Preparation of engineering reports and repair alternatives
• Cost estimating and constructability review
• Permitting and coordination with NC Dam Safety and other agencies
• Preparation of construction plans and specifications
• Construction-phase engineering support and inspections
All work shall be performed under the supervision of a Professional Engineer licensed in North
Carolina with demonstrated experience in dam repair and rehabilitation projects.
7. DETAILED SCOPE OF WORK
A. Evaluation & Engineering Services
Respondents shall demonstrate experience providing the following dam-specific services:

1. Review of existing dam documentation and prior inspections
2. Field investigations, including geotechnical and structural assessments
3. Hydrologic and hydraulic modeling consistent with dam safety criteria
4. Identification and evaluation of repair alternatives
5. Preparation of engineering reports suitable for regulatory review
6. Development of preliminary and final construction documents
7. Opinion of probable construction cost
B. Permitting & Regulatory Coordination
• Coordination with North Carolina Dam Safety Program
• Coordination with the FEMA Public Assistance Program
• Preparation of permit applications and responses to agency comments
• Support for environmental compliance, if required
C. Construction Phase Services
• Construction observation and periodic inspections
• Review of contractor submittals and pay applications
• Engineering support during construction and closeout
8. EVALUATION CRITERIA
Qualifications will be evaluated using the following criteria (100 points total):
1. Firm Experience with Dam Engineering & Repair – 30 points
2. Qualifications of Key Personnel – 25 points
3. Relevant Project Experience – 25 points
4. Methodology & Technical Approach – 10 points
5. References – 10 points
The Owner will rank firms and initiate negotiations with the highest-ranked respondent.
9. SUBMITTAL FORMAT

• Maximum length: 20 pages, excluding cover and divider pages
• Standard 8.5” x 11” format
• Clearly labeled sections corresponding to the RFQ requirements

Minimum insurance requirements shall include Professional Liability coverage appropriate for
dam engineering services.
11. SELECTION & NEGOTIATION
Crystal Lake Support, Inc. will negotiate scope and fee with the highest-ranked firm. If
negotiations are unsuccessful, the Owner may terminate negotiations and proceed to the next
ranked firm.
12. ANTICIPATED CONTRACT TERM
The anticipated contract term will be project-specific, with optional extensions as required to
complete permitting, design, and construction support.

structure to ensure statutory compliance and proper qualifications-based selection for dam
engineering services.
