A bus with a Driver is Needed for Non-Stop Round Trip Tours. The bus and assigned driver MUST Remain at Designated Site for the

Comprehensive General Liability Insurance.
The comprehensive general liability insurance policy (CGL) shall: (1)be occurrence type; (2) name City as an additional insured' (3) have
limits of not less than a combined single limit of Two Million Dollars (2,000,000.00) for any one incident; and (4)primary with respect to
Contractors general liability, notwithstanding any other insurance covering City. Business Automobile Liability Insurance. The business
automobile liability insurance policy shall cover each automobile, truck or other vehicle used in the performance of services in an amount not
less than a combined single limit of Five Million Dollars ($5,000,000.00) for bodily injury (including death at any time occurring) and
property damage per occurrence - The policy or policies shall be with companies authorized to do business in the State of Ohio and rated A
or above by A.M. Best Company or its equivalent - Copy of Certificate of the Liability Insurance must be sent by mail or facsimile to the
Office of the Director of Public Utilities or its designee upon receipt of award, along with a copy of the company's current workman
compensation certificate. Failure to comply will result in the cancellation of the purchase order.

Price quoted shall be F.O.B. delivered to the place designated on purchase order. No other terms will be acceptable.
Delivery quoted must be stated in terms of work days after receipt of the order.
