57220 Ohio Dept of Health Bureau of Infectious Diseases SFY 26 Long Term Care Infection Control Assessments

The goal for this project is to support Ohio skilled nursing facilities by utilizing IPC professionals to provide Infection Control Assessment and Response (ICAR) assessments and develop written recommendations and action plans to improve IPC practices to reduce HAIs and protect patients and healthcare personnel. The overall goal of the project is to provide proactive assistance and reduce future disease burden.

Offerors will need to review the attached RFP -  SRC0000031482 - 57220 Ohio Dept of Health Bureau of Infectious Diseases SFY 26 Long Term Care Infection Control Assessments. Each Offeror must submit a technical proposal and cost form/proposal as part of its total proposal before the closing date and time, which is 06/20/2025 at 3PM. If no qualifed bids are received, ODH reserves the right to post additional rounds to allow more time for qualified bidders to submit proposals.

The Offeror must title the electronic files with                                        “DOH RFP  # SRC0000031482/DOH57220 -- Technical Proposal” and "  - -DOH RFP  # SRC0000031482/DOH57220 Cost Proposal” as appropriate.

Please note there is not a page minimum or maximum for the technical proposal and bidders should refer to technical evaluation criterion on the RFP document for guidance.  The cost proposal should include the completed Excel Cost Proposal Form.
