It is the intent of Franklin County Municipal Court, Clerk of Court to obtain bids to establish a purchase order for envelopes for use in the Clerk’s Office, 375 South High Street, Columbus, Ohio 43215. NOTE: CITY PURCHASES MUST COMPLY WITH THE MAYOR'S EXECUTIVE ORDER (2007-02) REQUIRING THE USE OF RECYCLED PAPERS WITH A MINIMUM OF 30% POST CONSUMER FIBERS IN THE CONTENT. WHERE PRODUCTS ARE AVAILABLE THAT MEET THE REQUIREMENT, PLEASE PROVIDE QUOTES FOR THOSE PRODUCTS AND IDENTIFY THE PRODUCTS BEING QUOTED IN THE COMMENT AREA. Quantity: 15,000 LINE 20 – Accounting/Finance Division (Criminal Traffic) #10 standard, window envelope - the throat is no greater than 15/16 of an inch, sub 24, white wove envelopes with the return address printed, packed in 500 per box and 2500 per carton. Quantity: 10,000 LINE 30 – Civil Division #10 standard, window envelope - the throat is no greater than 15/16 of an inch, sub 24, white wove envelopes with the return address printed, packed in 500 per box and 2500 per carton. Quantity: 60,000 LINE 40 – Criminal/Traffic Division (Expungement) #10 standard, window envelope - the throat is no greater than 15/16 of an inch, sub 24, white wove envelopes with the return address printed, packed in 500 per box and 2500 per carton. Quantity: 7500 LINE 50 – Traffic Violations Bureau #10 standard, window envelope - the throat is no greater than 15/16 of an inch, sub 24, white wove envelopes with the return address printed, packed in 500 per box and 2500 per carton. Quantity: 7500 Delivery of the product will need to be delivered 2 weeks after the purchase order is approved, sent by fax or email and receipted by the winning bidder. Return address printed in black standard ink, diagonal seam, full glue flap. The Clerk’s Seal will be printed in two (2) colors. Proof required before mass production. Exact quantity pricing, no overruns or underruns. Shipping, handling and any artwork should be included in the final total bid price. Product shall meet all applicable U.S. Postal Service requirements and printing industry quality standards. The Seal and Return Address will be provided to the winning bidder. Each line number will have a different return address printed. DELIVERY: Bid shall include cost of required inside delivery, F.O.B. destination prepaid and allowed. Delivery Time: 9:30 am to 1:00 pm, Monday thru Friday, except Holidays. Dock Location: Loading dock is the first door off of South High Street on Fulton Street. Line 50. - Deliver To: Traffic Violations Bureau, 375 South High Street, 1st Floor, Columbus, Ohio 43215. Truck Size: Maximum truck size is 24 foot box truck (including cab). Lift Gate may be required. No fork lifts, or hand jacks are available in the building. Product must be unloaded on the dock and hand delivered.
