We hereby submit our proposal for the above referenced project. Enclosed please find
our proposal surety in the amount of ten percent (10%) of our total proposal. We have
reviewed and fully understand the Specifications, Request for Proposal, and the
General Information to Proposers, and we submit this proposal in complete compliance
with all stated conditions and requirements.
Upon award of the Contract, we will provide the required Certificate of Insurance issued

We hereby propose to perform all environmental testing, demolition, removal, and
satisfactory disposal of the residential dwelling structure located at 60 Helwig Street,
Milford, Connecticut. This proposal includes all services and materials associated with
the demolition, including but not limited to the removal and disposal of all contaminated
and hazardous materials, disposal fees, equipment costs, labor, grading, gravel,
permits, bonds, and insurance. Additionally, this proposal includes the complete repair
of the existing garage, inclusive of all required materials, labor, permits, bonds,
insurance and associated costs necessary to complete the scope of work.

4) Certificate of Insurance, together with endorsements, for the following will be
required upon award of contract and must remain in effect during the term of the
Contract
a) Identify the City of Milford as a certificate holder
b) Name the City of Milford, its Governing Board, Officials, Agents and
Employees as an additional insured.
c) Include the language "on a primary, non-contributory basis" and "waiver of
subrogation applies in favor of the City of Milford and all other required
parties."
