Event Description

The South Dakota Department of Health (SDDOH), Office of Chronic Disease Prevention and Health Promotion (OCDPHP) is seeking proposals from qualified agencies to support the South Dakota Heart Disease & Stroke Program. The successful applicant will provide guidance and support to the Cardiovascular Collaborative by facilitating both the Collaborative and its Leadership Team. In this role, the applicant will offer technical assistance and strategic support as members implement and evaluate interventions aligned with the statewide cardiovascular strategic plan. The work also involves leading the development of a new cardiovascular strategic plan for implementation in 2027, supporting workgroups within the Collaborative, and assisting with the ongoing updating, implementation, tracking, and communication of the Collaborative’s strategic plan. Additionally, the applicant will develop and maintain partnerships that enhance coordination and leverage resources across public health and state and local organizations.
