NON-PERSONAL SERVICE CONTRACT FOR RESPIRATORY THERAPIST

The Indian Health Service, Shiprock Service Unit, Northern Navajo Medical Center, located at US Hwy 491 North, Shiprock, New Mexico 87420 has a requirement is for 1 (one) Respiratory Therapist. Services will be performed in accordance to the attached Statement of work.

Period of Performance: Estimated from 12/16/2024 to 12/15/2025 – One (1) Year

The North American Industry Classification System or NAICS code is 621399 – Office of All Other Miscellaneous Health Practitioners with a small business size standard of $41.5M

The Request for Quotations No. IHS1503201 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be used. Evaluation factors may include technical capability to meet the government need outlined in the statement of work, past performance, and price.

Quotes will be due on or by November 26, 2024. The acquisition is a set aside as Indian-Owned Small Business.

In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
