ADDENDUM NO. 2

Date: January 30, 2026

This Addendum is issued to clarify and supplement the Request for Qualifications

(RFQ) documents. All other terms and conditions of the RFQ remain unchanged.

Proposers are responsible for incorporating this Addendum into their submittals.

Missing Required Forms

It has come to the Corporation’s attention that the following required forms were

inadvertently omitted from the original RFQ documents:

1. Disclosure of Interested Parties Form (Form 1295)

2. Statement of Non-Collusion

3. Certificate of Interested Parties / Conflict of Interest Certification (as

applicable)

These forms are hereby identified as required submission documents.

Submission Requirement

All respondents must complete and submit the above-referenced forms with their RFQ

response. Failure to submit the required forms may result in the proposal being deemed

non-responsive.

The forms are attached to this Addendum and will also be posted in the same location

where the original RFQ was advertised and made available.

Acknowledgment

Respondents shall acknowledge receipt of this Addendum in their RFQ submittal in

accordance with the instructions provided in the RFQ.
