seeking to prequalify trade partners for the Raleigh Police Department Evidence Facility project.
A non-mandatory information session will be held at 10:00AM Tuesday, July 21, 2026 at 2320
Law Enforcement Drive, Raleigh, NC 27610 in room 140.

Prequalification Form for Subcontractors
Pursuant to the statute, this form gathers information about the subcontractors seeking to
qualify for the work and provides a general format for the prequalification criteria. Completing
this questionnaire does not guarantee prequalification. Evaluation of the submittal shall be
performed by the prequalification committee in accordance with GS 143-128.1, 143-135.8 and
the City of Raleigh Prequalification Policy.
Subcontractors must use this project-specific form from the Prequalification Committee. Do
not use a blank template from the North Carolina State Construction Office website or any other
template previously used.

Project Description:
The Evidence facility consists of 2 ground up structures. The main building is load bearing
masonry with brick facade, with administrative office areas and secured storage areas
throughout. The second building is an enclosed PEMB with SOG for auto storage. Site to include
geothermal wells, retaining walls, utilities and underground storm detention.
Anticipated Start of Construction: November 2026
Submittals: Interested Firms must submit one (1) hard copies of this form and all required
attachments. Late submittals will not be accepted.
City of Raleigh Prequalification Policy: Please note that the City's prequalification policy has been
included in this document as Appendix C. First tier subcontractors unable to meet bonding
requirements shall be ineligible for further consideration in the prequalification process. If the
notary certification of this form is incomplete, the form is invalid.
