Process

The State of Ohio - Department of Administrative Services (DAS) is soliciting suppliers for concrete leveling and repair services.

Suppliers may respond to this solicitation after attending the MANDATORY site visit scheduled for 10:00am on Monday April 28th, 2025, At 4200 Surface Rd. Columbus Ohio, 43228. Please meet in the front lobby and sign in.
The contact for the site visite will be Jonathan Baker ( Jonathan.Baker@das.ohio.gov ) – Facility Manager or
David Straquadine ( David.Straquadine@das.ohio.gov ) – Chief Engineer

Please be sure to include the state contract number in the contract column of the grid, if applicable.

Completion of the grid is required and will be considered the response for evaluation. No outside/additional documentation will be considered.

The State Standard Terms and Conditions apply to this solicitation. No additional terms and conditions will be accepted.

Selection will be based on the lowest responsive and responsible quote who can complete work by June 30, 2025.

DAS may execute a written contract with the awarded supplier.
