I. INVITATION TO BID
Sealed bids for the provision/installation of one (1) State Building Code constructed modular house will be received at the Wayne County Courthouse, Finance Department, 224-226 East Walnut Street, 4th Floor, Goldsboro, NC 27530 on Thursday, August 27, 2026 at 3:00 pm and then will be publicly opened and read aloud. Proposals may be submitted to Mandy Trujillo, Procurement Specialist, Finance Dept., Wayne County Court House, 224-226 East Walnut Street, 4th Floor, Goldsboro, NC 27530 or delivered to same prior to the bid opening. Wayne County has received $750,000 in federal 2021 CDBG-NR grant funds representing 100% of total project costs to undertake housing rehabilitation, clearance, and reconstruction (replacement housing on site) activities. All federal CDBG requirements will apply to the Contract. The successful bidder on this work will be required to comply with federal regulations: Section 109 and E.O. 11246 which prohibits discrimination in employment regarding race, creed, color, sex, or national origin; Title VI of the Civil Rights Act of 1964; Anti-Kickback Act; E-Verify Regulations; and Contract Work Hours and Safety Standards Act. Wayne County is committed to and supportive of efforts to effectively maintain and/or increase the use of Small and Minority/Women Owned Business and Historically Underutilized Businesses (HUB) contract participation for construction projects; services (including professional and consulting services); and commodities purchases. Wayne County is also committed to increase contractor participation in employment, training and contracting opportunities to low and very low-income persons in the county in accordance with Section 3 of the Housing and Urban Development Act of 1968, as amended.

II. RESPONSIBITIES OF INVOLVED PARTIES
As the recipient of the CDBG grant funds, Wayne County is responsible for the bid process, selecting the successful bidder, preparing contract documents, monitoring work in progress, reviewing and approving invoices, issuing checks for approved work, monitoring contractor compliance with state and federal grant regulations, and approving contract completion activities.
The Contract will be between the Homeowner and the Contractor. The County will serve as administrator of the Contract and representative of the Homeowner, will work closely with the Homeowner, monitor the progress of contract activities, and issue payments for completed work.
III. STANDARD TERMS AND CONDITIONS
1. Payment Terms. Payment terms are Net 30 days after receipt of applicable invoice.
2. Compliance with All Laws. Contractor warrants that all performance hereunder shall be in accordance with all applicable federal, state and local laws, regulations and orders. If the source of funds for this contract is federal funds, the following federal provisions apply pursuant to 2 C.F.R. § 200.326 and 2 C.F.R. Part 200, Appendix II (as applicable): Equal Employment Opportunity (41 C.F.R. Part 60); Davis-Bacon Act (40 U.S.C. 3141-3148); Copeland “Anti-Kickback” Act (40 U.S.C. 3145); Contract Work Hours and Safety Standards Act (40 U.S.C. 3701-3708); Clean Air Act (42 U.S.C. 7401-7671q.); the Federal Water Pollution Control Act (33 U.S.C. 1251-1387); Debarment and Suspension (Executive Orders 12549 and 12689); Byrd Anti-Lobbying Amendment (31 U.S.C. 1352); Procurement of Recovered Materials (2 C.F.R. § 200.322); and Record Retention Requirements (2 CFR § 200.324).
3. E-verification. Contractor and any of its subcontractors must comply with the requirements of the North Carolina General Statutes, if applicable, which requires certain employers to verify the work authorization of each newly hired employee through the federal E-Verify program operated by the United State Department of Homeland Security and other federal agencies.
4. Iran Divestment Act Certification. Contractor hereby certifies that Contractor, and all subcontractors, are not on the Final Iran Divestment List (“List”) created by the North Carolina State Treasurer pursuant to NCGS 147, Article 6E. Contractor shall not utilize any subcontractor that is identified on the List.
5. Indemnification. Contractor shall indemnify and hold harmless Wayne County, its officers, agents, employees and assigns from and against all claims, losses, costs, damages, expenses, attorneys’ fees and liability that any of them may sustain (a) arising out of Contractor’s failure to comply with any applicable law, ordinance, or regulation or (b) arising directly or indirectly out of Contractor’s performance or lack of performance of their terms and conditions of the Contract. In event Contractor, its employees, agents, subcontractors and or lower-tier subcontractors enter premises occupied by or under the control of Wayne County in the performance of the Contract Documents, Contractor agrees that it will indemnify and hold harmless Wayne County, its officers, agents, employees and assigns, from any loss, costs, damage, expense or liability by reason of property damage or personal injury of whatsoever nature or kind arising out of, as a result of, or caused by the negligence or willful misconduct of such entrant.
6. Insurance. Contractor certifies that it currently has and agrees to purchase and maintain during its performance under this contract the following insurance from one or more insurance companies acceptable to Wayne County and authorized to do business in the State of North Carolina.
a. Worker’s Compensation: Coverage to apply for all employees for statutory limits in compliance with the applicable state and federal laws. The policy must include the employers’ liability with a limit $1,000,000 each accident.
b. Commercial General Liability: Shall have minimum limits of $1,000,000 per occurrence combined single limit for bodily injury liability and property damage liability.
c. Commercial Auto Liability: Shall have minimum limits of $1,000,000 each accident combined single limit for bodily injury liability and property damage liability. This shall include owned vehicles, hired and non-owned vehicles, and employee non-ownership.
d. Builders Risk Insurance: Contractor shall provide Builders Risk Insurance, naming the Homeowner as an additional insured. Builders Risk insurance shall cover the value of the modular housing unit from the time the unit arrives at the site to the completion of the installation and final payment of the unit. Upon final payment the Homeowner will provide insurance coverage for the housing unit.
e. Contractor shall furnish a copy of an original Certificate of Insurance, naming the Homeowner as an additional insured, to the County. Should any of the policies be canceled before expiration date, the issuing company will provide by mail thirty (30) days written notice to the certificate holder. Contractor shall furnish insurance in satisfactory limits, and on forms and of companies which are acceptable to Wayne County and shall require and show evidence of insurance coverage on behalf of any subcontractors (if applicable), before entering any agreement to sublet any part of the work to be completed under this contract.
7. Termination for Convenience. In addition to all of the other rights that Homeowner may have to cancel this Agreement, Homeowner shall have the further right, without assigning any reason therefore, to terminate any work under the Contract Documents, in whole or in part, at any time by providing written notice to Contractor. If the Contract is terminated by Homeowner in accordance with this Paragraph, Contractor will be paid for services actually provided up through the date of termination at the rates provided herein.
8. Termination for Default. Homeowner may terminate this Contract, in whole or in part, for failure of Contractor to perform any of the provisions hereof. In addition to any other remedies available to Homeowner in law or equity in connection with an uncured breach of the Contract by Contractor, Homeowner may procure upon such terms as the Homeowner shall deem appropriate, services substantially similar to those so terminated, in which case Contractor shall be liable to the Homeowner for any excess costs for such similar supplies or services and any expenses incurred in connection therewith.
9. Contract Funding. It is understood and agreed between Contractor and Wayne County that Wayne County’s obligation for payments is contingent upon the availability of appropriated CDBG grant funds from which payment for Contract purposes can be made. No legal liability on the part of Wayne County for any payment may arise until funds are made available to Wayne County’s Finance Officer and until Contractor receives notice of such availability. Should such funds not be appropriated or allocated, the Contract shall immediately be terminated. Wayne County shall not be liable to Contractor for damages of any kind (general, special, consequential or exemplary) as a result of such termination.
10. Contract Modifications. The Contract may be amended only by written amendment duly executed by both Homeowner and Contractor and approved by County. However, minor modifications may be made by Wayne County’s CDBG Project Manager to take advantage of unforeseen opportunities that: (a) do not change the intent of the Contract or the scope of Contractor’s performance; (b) do not increase Contractor’s total compensation or method of payment; and (c) either improve the overall quality of the product or service to Homeowner and Wayne County without increasing the cost, or reduce the total cost of the product or service without reducing the quantity or quality. All such minor modifications to the Contract must be recorded in writing and signed by both the Homeowner and Contractor, and placed on file with the Contract. No price adjustments will be made unless the procedure has been included in the Contract and a maximum allowable amount stipulated.
11. Independent Contractor. Contractor is an independent contractor and not an employee of Wayne County. The conduct and control of the work will lie solely with Contractor. The Contract shall not be construed as establishing a joint venture, partnership or any principal-agent relationship for any purpose between Contractor and Wayne County. Employees of Contractor shall remain subject to the exclusive control and supervision of Contractor, which is solely responsible for their compensation.
12. Permits and Licenses. Contractor will, at his own expense, obtain all necessary permits, give all notices, pay all license fees and comply with all laws, rules, ordinances, and regulations relating to the preservation of the public health or applicable to the services or business carried on under this contract. The burden of determining applicability of licensing requirements, laws, ordinances, and regulations for the Contractor and his employees rests with the Contractor.
13. Nondiscrimination. During the performance of the Contract, Contractor shall not discriminate against or deny the Contract’s benefits to any person on the basis of sexual orientation, national origin, race, ethnic background, color, religion, gender, age, or disability.
14. Conflict of Interest. Contractor represents and warrants that no member of Wayne County or any of its employees or officers who may obtain a direct benefit, personal gain or advantage for themselves or a relative or associate as a result of the Contract, subcontract or other agreement related to the Contract is in a position to influence or has attempted to influence the making of the Contract, has been involved in making the Contract, or will be involved in administering the Contract. Contractor shall cause this paragraph to be included in all Contracts, subcontracts and other agreements related to the Contract.
15. Gratuities to Wayne County. The right of Contractor to proceed may be terminated by written notice of Homeowner and Wayne County if the County determines that Contractor, its agent or another representative offered or gave a gratuity to an official or employee of Wayne County in violation of the policies of Wayne County.
16. Kickbacks to Contractor. Contractor shall not permit any kickbacks or gratuities to be provided, directly or indirectly, to itself, its employees, subcontractors or subcontractor employees for the purpose of improperly obtaining or rewarding favorable treatment in connection with the Contract or in connection with a subcontract relating to the Contract. When Contractor has grounds to believe that a violation of this clause may have occurred, Contractor shall promptly report to Wayne County in writing the possible violation.
17. Monitoring and Evaluation. Contractor shall cooperate with Homeowner and Wayne County, or with any other person or agency as directed by Wayne County, in monitoring, inspecting, auditing or investigating activities related to the Contract. Contractor shall permit Homeowner and Wayne County to evaluate all activities conducted under the Contract. Homeowner has the right at its sole discretion to require that Contractor remove any employee of Contractor from Homeowner property and from performing services under the Contract following a provision of notice to Contractor of the reasons for Homeowner’s dissatisfaction with the services of Contractor’s employee.
18. Financial Responsibility. Contractor is financially solvent and able to perform under the Contract. If requested by Wayne County, Contractor agrees to provide a copy of its latest audited annual financial statements or other financial statements as deemed acceptable by Wayne County’s Finance Officer. In the event of any proceedings, voluntary or involuntary, in bankruptcy or insolvency by or against Contractor, the inability of Contractor to meet its debts as they become due or in the event of the appointment, with or without Contractor’s consent, of an assignee for the benefit of creditors or of a receiver, then Homeowner shall be entitled, at its sole option, to cancel any unfilled part of the Contract without any liability whatsoever.
19. Dispute Resolution and Contract Situs. This Agreement shall be governed and will be construed in accordance with the laws of the State of North Carolina. Each party agrees that exclusive venue for all actions, relating in any manner to this Agreement will be in the General Court of Justice in Wayne County, North Carolina.
20. No Third Party Benefits. The Contract shall not be considered by Contractor to create any benefits on behalf of any third party. Contractor shall include in all contracts, subcontracts, or other agreements relating to the Contract an acknowledgment by the contracting parties that the Contract creates no third party benefits.
21. Force Majeure. “Force Majeure” shall mean any delays caused by acts of God, riot, war, terrorism, inclement weather, labor strikes, material shortages and other causes beyond the reasonable control of the affected party. If either party is unable to perform its obligations or in the case of Homeowner and Wayne County, to accept the Services because of Force Majeure, the time for such performance by such party or in the case of Homeowner and Wayne County, acceptance of Services will be equitably adjusted by allowing additional time for performance or acceptance of services equal to any periods of Force Majeure.
22. General Provisions. Homeowner’s remedies as set forth herein are not exclusive. Any delay or omission in exercising any right hereunder, or any waiver of any single breach or default hereunder, shall not be deemed to be a waiver of such right or of any other right, breach, or default. If Homeowner should prevail in any action instituted by Contractor hereunder, Homeowner shall be entitled to recover costs and reasonable attorney’s fees. Contractor may not assign, pledge, or in any manner encumber Contractor’s rights under this Order, or delegate the performance of any of its obligations hereunder, without Homeowner’s prior, express, written consent.
23. Entire Contract. This Contract constitutes and expresses the entire agreement and understanding between the parties concerning the subject matter of this Contract. This document, including exhibits, if any, any purchase order used in connection with this Contract and any other document expressly incorporated in this Contract by reference supersede all prior and contemporaneous discussion, promises, representations, agreements, and understandings relative to the subject matter of this Contract.
IX. ADDITIONAL TERMS AND CONDITIONS
A. NON-COLLUSION AFFIDAVIT
Each qualification package must be accompanied by a notarized affidavit on non-collusion, executed by the firm or in the case of a corporation, by a duly authorized representative of said corporation. The Non-Collusion Affidavit is provided herein.
B. ADDENDA/CHANGES
Any additions, deletions, modifications or changes made to this Request for Bids shall be processed through the Wayne County Finance Department. Any deviation from this procedure may result in the disqualification of the firm’s submittal or the cancellation of any contract resulting from this Request for Bids.
X. CONTACT POLICY
After the date and time established for receipt of proposals by the County, any contact initiated by any firm with any County representatives, other than the Purchasing Manager listed herein, concerning this Request for Bids is prohibited. Any such unauthorized contact may cause the disqualification of the firm from this qualification process.

• Install minimum 6’ x 8’ suspended corrugated concrete porch with brick steps and handrails at front entrance. Install minimum 4’ x 6’ concrete porch with brick steps and handrails at rear entrance. Porches to include vinyl railings, handrails, pickets, vinyl sleeve over pressure treated 4” x 4” posts at steps and porches. Install vinyl handrails, pickets on both sides of steps at all entrances.
• If client desires, pressure treated lumber decks may be substituted minimum 8’ x 10’ at front entrance and 6’ x 8’ at rear entrance with pressure treated steps, handrails, railings, pickets, posts, etc. Handrails shall be made of pressure treated 4” x 4” posts and 2” x 4” rails with rails at top and mid-height of posts. Handrails to be anchored securely in ground. Pickets shall be pressure treated 2”x 2” 4” apart. Railings at decks shall be made of pressure treated 4” x 4” posts and 2” x 4” rails with 2”x 2” pickets 4” apart. Install treated handrails on both sides of steps at front and rear entrances.
• Baths to have porcelain sink. Master Bath to have one piece fiberglass shower stall with door. Master Bath to have handicap commode. Grab bars to be installed inside and outside of shower stall and at commode. Hall Bath to have one piece fiberglass tub/shower. Hall Bath to have handicap commode. Grab bars to be installed outside of fiberglass tub/shower, and at commode.
• Kitchen to have stainless steel sink. No plastic plumbing fixtures in baths or kitchen.
• All bedroom doors and bathroom doors to have privacy locks. All interior doors to be raised panel doors. Master bedroom and master bath interior doors to have 36’ inch doorway. Exterior steel 6-panel front door with storm door. Exterior cottage inswing rear door (9-lite) with storm door at rear entrance. Both exterior doors to have 36” inch doorway.
• Electrical service 200 Amp. To be provided and shall include, meter base, breaker panel and connection mounted to home in accordance with Electrical Code. Contractor to coordinate work with power company and Building Inspector.
• Modular Unit to be underpinned with brick on a continuous poured concrete footing 6” thick by 10” wide and bottom of footing is to be 12” below ground level. Louvered foundation vents to be provided according to State Building Code, along with metal foundation access door. Unit height above ground shall be a minimum height of 24”.
• Contractor shall connect unit to public water supply at water meter with new minimum Schedule 40 ¾” PVC water line, two outside spigots and cut-off valve in value box within 5’ of house. Water line to be buried up to connection to unit. Install foam insulation on all exposed water lines under dwelling.
• Contractor to connect unit to septic tank or sewer tap with new sewer line, as required by Code.
• Contractor to provide street number address on unit in accordance with Local E-911 requirements.
• Tongues, wheels and axles to be removed and retained by Contractor. Tongues/wheels/axles cannot be placed under unit prior to underpinning.
• Grade, seed and hay yard twenty feet (20’) around all sides of house. Insure positive drainage away from foundation of house. Grade, level, seed, hay any disturbed area of yard as a result of set up of new dwelling.
• Install concrete driveway from highway to front of home location, approximately 65’feet long by 12’ feet wide and 4” inches thick. Driveway area to be smooth and level prior to concrete installation. Install concrete sidewalk from concrete driveway to front steps approximately 20’ feet long x 4’ feet wide x 4” thick. Provisions in CDBG Contracts Regarding Federal Requirements, As Applicable
Appendix II – Title 2 CFR Part 200—Contract Provisions for Non-Federal Entity Contracts Under Federal Awards
In addition to other provisions required by the Federal agency or non-Federal entity, all contracts made by the non-Federal entity under the Federal award must contain provisions covering the following, as applicable.
(A) Contracts for more than the simplified acquisition threshold currently set at $150,000, which is the inflation adjusted amount determined by the Civilian Agency Acquisition Council and the Defense Acquisition Regulations Council (Councils) as authorized by 41 U.S.C. 1908, must address administrative, contractual, or legal remedies in instances where contractors violate or breach contract terms, and provide for such sanctions and penalties as appropriate.
(B) All contracts in excess of $10,000 must address termination for cause and for convenience by the non-Federal entity including the manner by which it will be effected and the basis for settlement.
(C) Equal Employment Opportunity. Except as otherwise provided under 41 CFR Part 60, all contracts that meet the definition of “federally assisted construction contract” in 41 CFR Part 60-1.3 must include the equal opportunity clause provided under 41 CFR 60-1.4(b), in accordance with Executive Order 11246, “Equal Employment Opportunity” (30 FR 12319, 12935, 3 CFR Part, 1964-1965 Comp., p. 339), as amended by Executive Order 11375, “Amending Executive Order 11246 Relating to Equal Employment Opportunity,” and implementing regulations at 41 CFR part 60, “Office of Federal Contract Compliance Programs, Equal Employment Opportunity, Department of Labor.”
(D) Davis-Bacon Act, as amended (40 U.S.C. 3141-3148). When required by Federal program legislation, all prime construction contracts in excess of $2,000 awarded by non-Federal entities must include a provision for compliance with the Davis-Bacon Act (40 U.S.C. 3141-3144, and 3146-3148) as supplemented by Department of Labor regulations (29 CFR Part 5, “Labor Standards Provisions Applicable to Contracts Covering Federally Financed and Assisted Construction”). In accordance with the statute, contractors must be required to pay wages to laborers and mechanics at a rate not less than the prevailing wages specified in a wage determination made by the Secretary of Labor. In addition, contractors must be required to pay wages not less than once a week. The non-Federal entity must place a copy of the current prevailing wage determination issued by the Department of Labor in each solicitation. The decision to award a contract or subcontract must be conditioned upon the acceptance of the wage determination. The non-Federal entity must report all suspected or reported violations to the Federal awarding agency. The contracts must also include a provision for compliance with the Copeland “Anti-Kickback” Act (40 U.S.C. 3145), as supplemented by Department of Labor regulations (29 CFR Part 3, “Contractors and Subcontractors on Public Building or Public Work Financed in Whole or in Part by Loans or Grants from the United States”). The Act provides that each contractor or subrecipient must be prohibited from inducing, by any means, any person employed in the construction, completion, or repair of public work, to give up any part of the compensation to which he or she is otherwise entitled. The non-Federal entity must report all suspected or reported violations to the Federal awarding agency.
(E) Contract Work Hours and Safety Standards Act (40 U.S.C. 3701-3708). Where applicable, all contracts awarded by the non-Federal entity in excess of $100,000 that involve the employment of mechanics or laborers must include a provision for compliance with 40 U.S.C. 3702 and 3704, as supplemented by Department of Labor regulations (29 CFR Part 5). Under 40 U.S.C. 3702 of the Act, each contractor must be required to compute the wages of every mechanic and laborer on the basis of a standard work week of 40 hours. Work in excess of the standard work week is permissible provided that the worker is compensated at a rate of not less than one and a half times the basic rate of pay for all hours worked in excess of 40 hours in the work week. The requirements of 40 U.S.C. 3704 are applicable to construction work and provide that no laborer or mechanic must be required to work in surroundings or under working conditions which are unsanitary, hazardous or dangerous. These requirements do not apply to the purchases of supplies or materials or articles ordinarily available on the open market, or contracts for transportation or transmission of intelligence.
(F) Rights to Inventions Made Under a Contract or Agreement. If the Federal award meets the definition of “funding agreement” under 37 CFR §401.2 (a) and the recipient or subrecipient wishes to enter into a contract with a small business firm or nonprofit organization regarding the substitution of parties, assignment or performance of experimental, developmental, or research work under that “funding agreement,” the recipient or subrecipient must comply with the requirements of 37 CFR Part 401, “Rights to Inventions Made by Nonprofit Organizations and Small Business Firms Under Government Grants, Contracts and Cooperative Agreements,” and any implementing regulations issued by the awarding agency.
(G) Clean Air Act (42 U.S.C. 7401-7671q.) and the Federal Water Pollution Control Act (33 U.S.C. 1251-1387), as amended—Contracts and subgrants of amounts in excess of $150,000 must contain a provision that requires the non-Federal award to agree to comply with all applicable standards, orders or regulations issued pursuant to the Clean Air Act (42 U.S.C. 7401-7671q) and the Federal Water Pollution Control Act as amended (33 U.S.C. 1251-1387). Violations must be reported to the Federal awarding agency and the Regional Office of the Environmental Protection Agency (EPA).
(H) Debarment and Suspension (Executive Orders 12549 and 12689)—A contract award (see 2 CFR 180.220) must not be made to parties listed on the governmentwide exclusions in the System for Award Management (SAM), in accordance with the OMB guidelines at 2 CFR 180 that implement Executive Orders 12549 (3 CFR part 1986 Comp., p. 189) and 12689 (3 CFR part 1989 Comp., p. 235), “Debarment and Suspension.” SAM Exclusions contains the names of parties debarred, suspended, or otherwise excluded by agencies, as well as parties declared ineligible under statutory or regulatory authority other than Executive Order 12549.
(I) Byrd Anti-Lobbying Amendment (31 U.S.C. 1352)—Contractors that apply or bid for an award exceeding $100,000 must file the required certification. Each tier certifies to the tier above that it will not and has not used Federal appropriated funds to pay any person or organization for influencing or attempting to influence an officer or employee of any agency, a member of Congress, officer or employee of Congress, or an employee of a member of Congress in connection with obtaining any Federal contract, grant or any other award covered by 31 U.S.C. 1352. Each tier must also disclose any lobbying with non-Federal funds that takes place in connection with obtaining any Federal award. Such disclosures are forwarded from tier to tier up to the non-Federal award.
(J) Sec. §200.322 Procurement of recovered materials.
SECTION 3 OF THE HOUSING AND URBAN DEVELOPMENT ACT OF 1968
Section 3 of the Housing and Urban Development Act of 1968 provides preference to low and very low-income residents of the community (regardless of race or gender), and the businesses that substantially employ these persons, for new employment, training, and contracting opportunities. Contractors doing work with Wayne County are to list any new hires with the local NCWorks Career Center and to report any new hires to the County. Firms are encouraged to provide preference to low-income community residents and residents in public housing units. Attached is information about Section 3.The following Section 3 clause will be included in all CDBG contracts where Section 3 applies.
A. The work to be performed under this Contract is subject to the requirements of Section 3 of the Housing and Urban Development Act of 1968, as amended (“Section 3”). The purpose of Section 3 is to ensure that employment and other economic opportunities generated by HUD assistance or HUD-assisted developments covered by Section 3, shall, to the greatest extent feasible, be directed to low- and very low-income persons including persons who are recipients of HUD assistance for housing, with preference for both targeted workers living in the service area or neighborhood of the Development and Youthbuild participants, as defined at 24 CFR Part 75 (“Section 3 Regulations”).
B. The Parties agree to comply with HUD's regulations in Section 3 Regulations, which implement Section 3. As evidenced by their execution of this Contract, the Parties certify that they are under no contractual or other impediment that would prevent them from complying with the Section 3 Regulations.
C. The Contractor agrees to send to each labor organization or representative or workers with which the contractor has a collective bargaining agreement or other understanding, if any, a notice advising the labor organization or workers’ representative of the contractor’s commitments under this Section 3 clause and will post copies of the notice in conspicuous places at the work site where both employees and applicants for training and employment positions can see the notice. The notice shall describe the Section 3 preference, shall set forth the following: (i) minimum number and job titles subject to hire, (ii) availability of apprenticeship and training positions, (iii) qualifications for each, (iv) name and location of the person(s) taking applications for each of the positions, and (v) the anticipated date the work shall begin.
