The City reserves the right to reject a bid if is not complete according to the bid instructions.
BID INSTRUCTIONS

To submit a project bid you must attend the walk-through and sign in. The CDBG Administrator
and/or Project Manager will be present to discuss any problems and/or questions you have relating to this
project. Addendums will be prepared if warranted and posted under “Changes in Bids” on the Purchasing
Department webpage https://www.ci.milford.ct.us/purchasing-department
The City will not accept or consider bids, withdrawals, or amendments received after the date and time
specified. Bids sent by FAX or Email alone are not acceptable and will not be considered. Complete in
pen or
typed, no pencil entries accepted. If a complete bid packages is equal in price, quality, and service, the
bidder who includes a drug free workplace certification form will be given preference in accordance with
Milford's Code of Ordinances, Article V, Division 2.
In reviewing the qualifications of a bidder, the City may consider a company’s previous performance
record and quality of service on contracts for similar work; and the City expressly reserves the right to
reject the bid of such bidder if such record discloses that such bidder, in the opinion of the City, has not
properly performed such contracts or has habitually, and without just cause, neglected the payment o
bills or has otherwise disregarded their obligations as said bidder.
The City is exempt from State sales tax and from Federal excise tax - do not include these taxes in your
proposal.

will adhere and comply to Federal, State, local regulations as noted herein.
Bid Form: The bid will be accepted with the clear understanding that no revisions can be made to the
price after it is submitted.

Contractor License - State of Connecticut Home Improvement Contractor or a Trade Business
License(s).
Conflict of Interest - Submit the conflict of interest statement form with the estimate. Disclose whether
anyone associated directly with the company, including a family member or a local official will benefit from
accepting federal funding to complete the project.
Drug Free Workplace Certification This form is optional unless estimates cost are tied. Completed by
typewriter or handwritten in ink no pencil entries will be accepted.
W-9 Finance Department requires a W-9 for tax purposes; include D/B/A.
Reference Submit a minimum of one (1) professional reference related to the work specifications.
GENERAL CONDITIONS
The Contractor/Vendor agrees to provide the following documentation prior to signing contract and
receiving a purchase order.
1. Upon bid award, the Contractor/Vendor will provide a Certificate of Insurance showing minimum
coverage of $100,000/300,000 personal liability and $30,000 property damage, or an amount
commensurate with the project cost, and Workmen Compensation coverage. Subcontractors, if
allowed, must submit a Certificate of Insurance with the same coverage.
2. The Contractor/Vendor will file a 100% Performance Bond and a 100% Labor & Materials Bond with
Purchasing Department or CDBG Administrator, Department of Economic & Community
Development, if required.
3. The Contractor/Vendor will execute a Notice to Proceed form when provided by the City, signed and
dated by both the parties. It will state a mutually convenient date to begin and end the project.
4. Contractor/Vendor is required to obtain a registration in the System for Award Management (SAM) to
receive federal funds through HUD CDBG program. all organizations that conduct business with the
U.S. Federal government to obtain a Dun and Bradstreet Data Universal (DUNS) number and register
in the System for Award Management (SAM).
Assistance is FREE to register in both Dun & Bradstreet and System for Awards Management.
Obtain a DUNS number to set up a new or renew a SAM registration. To obtain/search/update a
DUNS number go to http://fedgov.dnb.com/webform D&B D-U-N-S Number for US Government

5. The successful bidder will comply with the provisions of the Davis- Bacon and Related Acts regarding
the labor requirements for non-residential construction projects or residential contracts over $25,000.
Construction activity subject to these provisions will be managed in accordance with the Construction
Contract Provisions of Labor Standard Clauses outlined in Federal Labor Standards Provisions HUD-
4010. Adherence to Davis-Bacon Wage Decisions Prevailing Wage Rates in Appendix 1 requires
submission of a Payroll Summary on DOL Form WH-347 with each payment request.
6. The Contractor/Vendor shall employ Abatement and Lead Safe Work Practices (LSWP) licensed and/
or certified staff to supervise the project on a daily basis. All work should be performed in
conformance with the EPA regulations and HUD Lead-Based Paint Poisoning Prevention Regulations
(24 CFR Part 35).
7. The construction specifications for this property shall comply with the provisions of Circular HPMS-
FHA 4500.5 as it relates to the prohibition of the use of Lead Base Paint. Only paint that conforms to
the American Standards Institute Z66.L (1964).
8. Submit Contract and Subcontract Activity HUD form 2516 to make record of sub-contractors hired to
complete the project. Update list when necessary.
9. The Contractor/Vendor will provide the CDBG Administrator/Project Manager with copy(s) of the
applicable local building permit(s) or state permits, if applicable, BEFORE work may commence.
10. All work specified shall be performed according to the local building and zoning codes.
11. The contractor/bidder shall familiarize himself with all specifications If inadequate or unforeseen
conditions are found prior to the bidding the contractor/vendor shall advise the CDBG
Administrator/Project Manager or Owner of condition immediately.
12. In case of ambiguities and inconsistencies between sections, greater quantities and better quality
shall take precedence over lesser quantities and qualities.
13. The Contractor/Vendor shall guarantee all work performed for a period of 1 year from the date of
completion. All material warranties shall be given to the owner.
14. The Contractor/Vendor shall provide all material and labor to correct the deficiency. Damage to or
failure of work due to abuse, misuse or other unusual circumstances shall be the responsibility of the
Contractor to correct.
15. All work shall be inspected by the Milford Building Department, if applicable, and/or the Project
Manager.
16. All plumbing fixtures shall comply with water regulation codes.
17. In addition, requirements of the Local Building and Fire Departments must be adhered to and
followed as required. It shall be the duty of the contractor to request an inspection from the Building
Department before closing any walls or ceilings.
18. Electrical contractors are to use wire mold only in severe circumstances. Before installing wire mold,
the contractor must notify the Building Department and the Project Manager, if applicable.
19. The Contractor/Vendor shall request use of, at no cost, existing utilities such as electric lights, heat,
electrical power and water necessary to carry out and complete the work.
20. The Contractor/Vendor shall be responsible for completion of the entire scope of work as specified.
Any proposed change order must be authorized by the CDBG Administrator/Project Manager in
advance.
21. Contractor/Vendor agrees to provide quality workmanship using standard means and methods, and
complete work that is fit for inspection and approval, esthetically finished and operational for use.
22. There will be no final payment made until electrical, plumbing, and building permits have been
approved and Building Department inspection approvals have been received by the CDBG
Administrator.

SCOPE OF WORK
Please Note:
The following information is to be used in conjunction with specifications, project archived photos and pre-bid site
walk.
General:
The following is an outline of the Scope of Work. This Scope of Work includes all labor and materials required to
perform the construction work as described below. The contractor is responsible for obtaining permits; providing
complete, thorough, and compliant services to all work; and for coordinating as appropriate with the work of all
other trades specified elsewhere. All work must be performed in compliance with all applicable National, State,
and Local Codes.
Lead Disclosure:
This project is funded by the Department of Housing and Urban Development (HUD) and
requires the use of lead safe work practices under 24 CFR Part 35 (Lead Based Paint
Poisoning Prevention and Control in Certain Residential Structures). A Lead Inspection
and Risk Assessment may be required for this property.
*Please note that all material specifications referenced in the following scope outline are recommendations, and
that approved substitutes may be considered. Additionally, any items or scenarios not specifically mentioned in this
scope outline that may avail themselves during pre-bid walkthrough will be addressed via Addendum.
Scope Outline
The successful bidder shall furnish all labor, materials, tools, equipment, permits, and
supervision necessary to complete the following work:
DIVISION 06: WOODS AND PLASTICS (Breezeway)
• Remove existing corrugated sheeting material and framing.
• Remove existing storm doors and frames.
• Clean and prep concrete slab.
• Construct stud-wall framing on front (street side) and back (yard side) using pressure treated 2 x 4 lumber
(bottom plate) and SPF stud framing (16” O.C.). Framing should include rough framing for front and back
doors.
DIVISION 07: THERMAL AND MOISTURE PROTECTION
(Roof Scope Includes ALL Structures)
• Remove all existing shingles down to the roof deck.
• Inspect all decking for worthiness. Include allowance for 10 sheets of 1/," CDX plywood decking in the base bid.
Replace as needed.
• Provide per sheet pricing for additional decking. (*Note: Additional decking replacement beyond the allowance
will require prior approval and must be submitted as a Change Order.)
• Install Owens Corning WeatherLock® ice & water shield at all gutter lines.
• Install Owens Corning lnterwrap RhinoRoof® UDL Synthetic Underlayment on all remaining roof sections.
• Install Owens Corning Starter Strip Shingles along all gutter lines.
• Install Owens Corning TruDefinition® Duration® Limited Lifetime Dimensional Shingles per manufacturer
specifications using 1¼" roofing nails.
