Amendment 0001 is hereby issued to add H.15 - Transition of Services and Continuity of Care, H.16 - Quality Assurance Plan (QAP), Attachment11-Quality Assurance Plan, and 52.222 - 90 - Addressing DEI Discrimination by Federal Contractors (APR 2026).All other Terms, Conditions, and Line Items remain the same.

This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with part 12. This announcement constitutes the only solicitation. Offers are being requested and a separate written solicitation will not be issued.

Solicitation number 15BFA026R00000004 is issued as a Request for proposal (RFP) for Comprehensive Medical Services.

This acquisition is not set-aside for small business concerns. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov.

(iii) No brand name requirements for the items are to be acquired.

(iv) Services rendered will be provided at FMC Carswell located in Fort Worth, Texas. The date and times will be determined after award. Please see attached solicitation for details.
