The VA is seeking QUALIFIED VENDORS to provide a very brief response to this notice including only just what is asked for in this notice below. This is sources sought notice to industry and the only information is posted on Contract Opportunities web page at SAM.gov. Sources sought means seeking qualified vendors. BRIEF SCOPE OF WORK
(This is for information only)

IMPORTANT NOTE: This Sources Sought is for market research purposes only (to determine the socioeconomic status of interested companies). No award will be made off this posting.

The Grand Junction VA Health Care System (GRJ VAHCS) requires (8) powered, bariatric exam tables at their medical facility.

Salient Characteristics:

Exam table must be ADA compliant for a small exam room. (Exam rooms are approximately 10 x 10 and usually includes an exam bed, provider desk, and clinical work area.)

Table dimensions: **+/- 2 inches from the provided dimension is acceptable**

Table length expandable from 57 to 78 approximate when flat
Table width approx.. 28

Bed/exam table base should be approximately 46 length by 32 wide

Must include seamless upholstered/cushioned exam table tops
width approximately 28
Upholstery must be seamless, non-porous, and offer a protective finish to resist bacteria growth, staining, and damage.
Preferred color of upholstery: black

Must include leg extensions can extend 18 for patient in prone position

Table adjustments:
Must be able to adjust height from approx.. 18 (minimum) to approx.. 37 (maximum) **+/- 2 inches acceptable**

Table must be able to change from low flat position to up right sitting position and back support positioning with powered control

Back support angle: 0 degrees minimum and approx.. 80 degrees maximum

Pelvic tilt: 0 degree (horizontal) to 6 degree (up) approximate

Must include foot control for powered positioning (height adjustment, back, tilt and angle adjustments)

Base unit must include at least one (1) storage drawer
Approximately 9 x 19 x 4 to store a small amount of clinical supplies
Must be heated for OB/GYN or Urology use

Weight capacity: must be able to support up to 700 lbs.

Must include side safety handles (2 per table) that can lock into place to prevent left or right rotation. Should be able to rotate to the side or toward the back of the table for improved access and stability of the patient

Must include stirrups

Must be able to use with electrical outlets (plug with grounding pin)
Power requirements: 115-230 V depending on configuration

Safety features:
Cutoff sensor activates when obstruction is in the moving tracks
Safety switch on side of table to manually stop movement

The Contractor, its employees, agents, and subcontractors shall not be considered VA employees for any purpose of fulfilling the PWS and shall be considered employees of the Contractor. Contractor shall furnish all labor, materials, equipment, tools, shipping, transportation, insurances, licenses, certifications, and supervision necessary to provide services in accordance with this Performance Work Statement (PWS). The Contractor is responsible for all charges and fees related to disposal of materials generated by the Contractor.

This is anticipated to be a commercial supply acquisition under the authority of FAR Part 12, Simplified Acquisition Procedures for Commercial Items.

IMPORTANT NOTE: This RFI (Request for Information) is for information only.
No award will be made from this RFI.

QUALIFIED VENDOR: A qualified source has the a) necessary equipment, vehicles, personnel, resources and current operations as well as b) recent experience (within past 3 years) of performing services of a comparable size, scope and complexity, AND c) OEM certified technicians to perform services with unrestricted access to the facility during inclement weather, as well as immediate access to OEM software updates. A qualified source also has current and valid registration in System for Award Management (SAM), https://www.sam.gov). A qualified source will also be able to comply with Limitations of Subcontracting pursuant to FAR 19.507.
