FORSYTH COUNTY COOPERATIVE EXTENSION MOVE
Section I: GENERAL OVERVIEW, TERMS AND CONDITIONS
1. Purpose and Intent of the Request for Proposal (RFP)
Forsyth County (the "County") is requesting proposals from qualified Moving Contractors to provide a
complete package for moving of the Forsyth County Cooperative Extension from their current location at
1450 Fairchild Rd, Winston-Salem, North Carolina 27105, to their new location at 7232 Forsyth Center
Dr, Tobaccoville, NC 27050
2. Services Requested
Moving of the Cooperative Extension Office will include relocation of mainly contents with some FF&E
items. Types of FF&E included in move (but not limited to this list), storage cabinets, coat racks, storage
shelving.
Items handled by others and excluded from this contract are:
* Copiers
* Personal items (staff have been directed to move their personal items if sentimental or fragile)
The Move Contractor shall provide boxes or totes and labels to staff to load individual files, reference
books and other items for the move. The Move Contractor will be responsible for moving these items to
the new location.
All supplies, materials, and equipment required in this proposal must be new and in first class condition.
Bids offered on inferior or "second line" equipment will not be accepted. Exception: reusable totes if
used are not required to be new, but must be in good serviceable condition.
The Move Contractor shall have SOLE responsibility for accuracy of all measurements and estimates of
material quantities and sizes of moving containers.
The Move Contractor shall furnish all labor, equipment including vehicles, materials, permits, fees, waste
disposal, clean-up and shall abide by all local, state and federal codes applicable to moving the Forsyth
County Cooperative Extension as specified during the pre-bid conference, and outlined in the Move List
(Attachment II) and Move Plans (Attachment III).
Existing shelving shall be dismantled or otherwise packaged, transported and reassembled in accordance
with move plans. Furniture will need to be cleaned before reassembly in its new location.

No furniture, boxed material, etc will be allowed to be left overnight in the mover's truck.
Moving boxes may be returned to the Move Contractor for reuse or recycling at the owner's leisure after
unboxing of materials. If reusable totes are used, allow for a minimum of 2 weeks post move for staff to
unpack and have totes readied for pickup. Any totes or boxes not picked up after 30 days of being notified
are ready for pickup shall be deemed surplus and disposed of by Forsyth County in accordance with
county policies.
Proposal should reflect that move services may take place during or outside Normal Business Hours.
Normal Business Hours are defined as Monday-Friday, 8:00 am - 5:00 pm.

4. Mandatory Pre-Bid Conference and Site Visit
Conference and Site Visit is scheduled for Friday, April 24, 9:00am at 1450 Fairchild Rd, Winston-
Salem, NC 27105. All bidders are required to visit the job site prior to submitting bid to ascertain all
requirements of the job, measurements, materials needed, working conditions, etc. CONTACT: James
Anderson Jr at andersja@forsyth.cc for questions regarding the site visit. Site visit will include tour of the
move out location. New location is approximately 20-30 minutes from the existing location. Interested
bidders may tour the new location, 7232 Forsyth Center Dr, Tobaccoville, NC, 27050, Friday, April 24, at
1:30pm.
5. Questions and Clarification
Any and all questions or comments regarding this bid, the specifications, or scope of work, must be
submitted via email to andersja@forsyth.cc and copy imescp@forsyth.cc and simmonal@forsyth.cc. The
deadline for questions is 5:00 pm on April 29, 2026. Questions submitted after this time may not be
responded to.
Any questions and responses specific to the terms and conditions, process, procedures, language, specifications and
other parts of the RFP may be made public and may be shared with other Respondents in the form of an addendum
to the RFP. Questions and responses that contain proprietary information will be answered confidentially .
Questions and answers will be provided via e-mail. All addenda will be provided via e-mail. Therefore, no oral
statements by any person shall modify or otherwise affect the terms, conditions, or specifications stated in this RF P.

6. Special Provisions
A. Minimum Qualifications
Move contractor must have been in business for at least 5 years.
Move contractor must not have any current pending litigation without reasonable explanation.
Move contractor's managing office must be located within 75 miles of Winston-Salem, NC.
Move contractor must disclose if labor force is subcontracted.
B. Reservations
The County reserves the right to accept or reject any or all Proposals as a result of this request,
to negotiate with all qualified sources, or to cancel, in part or in its entirety, this Request for
Proposal if found in the best interest of the County. All Proposals become the property of
Forsyth County.
C. Insurance Terms
During the term of contract, the contractor shall comply with all applicable provisions of laws,
codes, ordinances, rules, regulations, and tariffs:
1. The winning contractor shall maintain such insurance authorized to do business in North
Carolina as will protect the contractor from the types of claims set forth below which may
arise out of, or result from the contractor's operations under the contract and for which the
contractor may be legally liable, whether such operations be by the contractor or by a Sub-
Contractor or by anyone directly or indirectly employed by any of them, or by anyone for
whose acts any of them may be liable:
a. Claims under workers' compensation, disability benefit, and other similar
employee benefit acts which are applicable to the work to be performed;
b. Claims for damages because of bodily injury, occupational sickness or disease, or
death of the contractor's employees;
c. Claims for damages because of bodily injury, sickness or disease, or death of
any person other than the contractor's employees;
d. Claims for damaged insured by usual personal injury liability coverage;
e. Claims for damages, other than to the work itself, because of injure to or
destruction of tangible property, including loss of use resulting thereof;
f. Claims for damages because of bodily injury, death of a person or property
damage arising out of ownership, maintenance or use of a motor vehicle;
g. Claims for bodily injury or property damage arising out of completed operations
including liability coverage for installation work performed by you or your Sub-
Contractors. Forsyth County must pre-approve all Sub-Contractors.; and
h. Claims involving contractual liability insurance applicable to the Contractor's
obligations under the provision entitled Indemnification - Third Party Claims.

2. Coverage shall be written on an occurrence basis and shall be maintained without
interruption from the date of commencement of the work until date of final payment.
Coverage must include the following on a commercial basis:
a. Premises - Operations,
b. Independent Contractor's Protective,
c. Products and Completed Operations,
d. Personal and Advertising Injury,
e. Contractual, including specific provision for contractor's obligations under the
provision entitled Indemnification - Third Party Claims,
f. Broad Form Property Damage including Completed Operations, and
g. Owned, Non-owned, and Hired Motor Vehicles.
3. The insurance required by this paragraph shall be written for not less than the following
limits of liability or as required by law, whichever coverage is greater:
COMMERCIAL GENERAL LIABILITY:
General Aggregate (per project) - $2,000,000
Products/Completed Operations - $1,000,000
Personal and Advertising Injury - $1,000,000
Each Occurrence - $1,000,000
Fire Damage (any one fire) - $50,000
Medical Expense - $5,000
BUSINESS AUTO LIABILITY
(including all owned, non-owned, and hired vehicles):
Combined Single Limit - $1,000,000
WORKERS COMPENSATION: $1,000,000
4. Required Documentation:
a. Prior to commencement of the work, contractor shall provide to the County of
Forsyth a signed, original certificate of liability insurance. The certificate shall
identify the types of insurance; state the limits of liability for each type of
coverage, include a provision for 30 day notice prior to cancellation, name Forsyth
County as a Certificate Holder, and list Forsyth County, its Officials, Officers,
and Employees as additional insured.
b. The County's failure to demand either a certificate of insurance or written
endorsement required by this section is not a waiver of contractor's obligations to
obtain the required insurance.

D. Safety of Persons and Protection of Property
Precaution shall be exercised at all times for the protection of persons, (including employees)
and property. Contractor shall protect against damage or interruption of service at all times by
the Contractor during the term of the Contract all furniture, floor covering and areas within
the work zone and areas outside of the work area. Existing structures, utilities, roads,
services, trees and shrubbery shall also be protected. The contractor shall assume all
responsibility to maintain all existing protection as required by the governing laws,
regulations, ordinances, and safety of personnel and visitors.
The Contractor assumes all risks of direct and indirect damage or injury to the property or
persons used or employed on or in connection with the work contracted for, and of all damage
or injury to any person or property wherever located, resulting from any action omission,
commission, or operation under the Contract, or in connection in any way whatsoever with the
contracted work. He shall make good in an approved manner at his own expense any such
loss, damage, or injury without cost to the owner. If the Contractor fails to make satisfactory
repairs the Owner will repair any damage by Contractor or his workmen and deduct cost from
the contract.
At the new facility, the awarded contractor shall provide floor protection, door protection, and
corner protection to protect the new finishes from damage. Any damages will be the
responsibility of the awarded contractor to pay Shelco LLC and/or their subcontractors to
repair.
E. Release
The Contractor assumes full responsibility for the work to be performed hereunder and hereby
releases, relinquishes, and discharges the County, its officers, agents, and employees from all
claims, demands, and causes of action of every kind and character, including the cost of
defense thereof, for any injury to or death of any person and any loss of or damage to any
property that is caused by, alleged to be caused by, arising out of, or in connection with the
Contractor's work to be performed hereunder. This release shall apply regardless of whether
said claims, demands, and causes of action are covered in whole or in part by insurance and
regardless of whether such injury, death, loss, or damage was caused in whole or in part by
the negligence of the County, any other party released hereunder, the Contractor, or any third
party.
7. Taxes, Fees, Code Compliance, Licensing
The moving contractor shall be responsible for payment of any required taxes or fees associated with the
contract. The contractor shall be responsible for compliance with all applicable codes and laws in
connection with performing the work contemplated under the contract.
A. Applicable North Carolina Sales and Use Taxes are NOT to be shown on bids, but are to be
added to invoices as a separate item.

The mover warrants that all goods, services or work supplied under this Agreement shall conform to
specifications, drawings, or other descriptions contained or referenced herein. The relocated products will
be inspected by the to insure that the items are in the same working condition as before the move. Neither receipt
of goods nor payment therefore shall constitute a waiver of this provision.
Move Contractor shall employ competent superintendent and necessary assistants who shall be in
attendance at the Project premises during performance of the work. The superintendent shall represent
the Mover, and communications given to the superintendent shall be binding as if given to the Mover.
Upon completion of all work covered in this specification the Contractor shall remove all equipment,
material, and debris leaving the area in an undamaged and acceptable condition. The Contractor shall be
responsible for all disposal fees.
Parking is available in the surface lots at both locations. Neither site has a loading dock, so the use of lift
gates and/or ramps will be required to complete the move.
9. Alcohol, Tobacco & Drug Rules and Regulations
Employees of the Move Contractor and its subcontractors shall comply with all instructions, pertaining to
conduct and building regulations of the Owners. The Owner reserves the right to request the removal or
replacement of any undesirable employee at any time.
All buildings on the Owner's grounds are tobacco-free. Use of tobacco products is not permitted in any
area inside Owner's buildings. The Move Contractor is expected to respect this tobacco-free policy and
fully comply with it. The Move Contractor agrees that in the performance of this Agreement, neither the
Move Contractor nor any of its employees shall engage in the unlawful manufacture, distribution,
dispensing, possession, or use of a controlled substance, including alcohol, in conducting any activity
covered by this Agreement. The County reserves the right to request a copy of the Drug Free Workplace
Policy. The Move Contractor further agrees to insert a provision similar to this statement in all
subcontracts for services required.
10. Forsyth County Rules and Regulations
* Eating and/or drinking shall not be allowed inside the Existing or New Locations.
* Move Contractor is responsible for cleaning up of interior and exterior areas on a daily basis as a
result of delivery and installation.
* Appropriate dress is required. All employees or representatives for the Move Contractor shall be
visibly identifiable as such by uniform and/or identification tag.
11. Failure to Perform
If the Move Contractor fails to perform as outlined herein, the County may terminate the contract at its
discretion and be liable only for the portion of acceptable work completed. The value of work completed
shall be the sole determination of the County in such case.
12. Payment

The Owner will make the payment in full after acceptance is given and the Move Contractor has
submitted a proper invoice and written warranties to the owner.
13. Project Schedule
The Move Contractor is responsible for the coordination of his work with the Owner in order to insure
timely completion of this project. All work shall be completed without interruption.
The Move Contractor shall report any problems regarding completion of servicesi mmediately to Forsyth County.
14. References and Proprietary Information
All Move Contractors grant the County permission to make inquiries concerning the respondent and its
qualifications and references to any persons or contractors deemed appropriate by the County. Any
proprietary information that the respondent provides in response to this RFP and for which contractor
does not want disclosed to the public shall be so identified on each page on which it is found. Data or
information so identified will be used by the County solely for the purpose of evaluation and contract
negotiations. Disclosure of any of contractor's proprietary information by the County to third parties shall
be in strict accordance with the laws and regulations regarding disclosure in the State of North Carolina.
15. Award
Forsyth County reserves the right to accept proposals, award proposals and/or not award proposals as a
whole; to reject any and all proposals, to waive any informality in the proposals, and to accept the
proposal that appears from all consideration to be for the best interest of Forsyth County.
Award will be made to the lowest responsible responsive bid received.
Notice of proposal award, if proposal be awarded, will be made within thirty (30) days of opening of
proposals. A fully executed contract between the Move Contractor and Forsyth County for the supplies,
materials, equipment or services as described in the Proposal will indicate the award of the proposal and a
contract to proceed.
Section II: BID REQUIREMENT
Proposals shall be mailed, hand delivered or emailed. Clearly mark on the outside of the sealed package
"Forsyth County Cooperative Extension Move Proposal". If submitting via mail, please make sure to
allow time for delivery from our central mail service. Mailed proposals must be received in the General
Services Office before the due date and time or will be deemed non-responsive.
Proposals will be accepted until 2:00 pm on May 21, 2026 at the address below.

not be accepted. Incomplete responses will be deemed non-responsive and will be rejected from
consideration. The County is not liable for any cost incurred by any person or firm responding to this
RFP.

It is the policy of the County of Forsyth that a County employee, officer or agent of the County may not
participate in personal services or construction in which contractor or subcontractor, or any member of his
immediate family, business partner or any organization in which they serve as an officer, director, trustee
or employee, has a financial interest.
By submitting a proposal, the successful Contractor agrees to indemnify the County from and against all
claims, suits, damages, costs, losses, and expenses in any manner arising out of or connected with the
failure of the Company, its subcontractors, agents, successors, assigns, officers, or employees to
comply with the provisions of the aforementioned regulations or the rules and regulations
promulgated there under any or all of same.
Pursuant to N.C.G.S. Chapter 55 Article 15 entitled Foreign Corporations, the successful Contractor must
have on file with the Secretary of State of the State of North Carolina a Certificate of Authority to transact
business in this state.
E-VERIFY
Per N.C.G.S. 143-133.3, CONTRACTOR shall comply with the requirements of Article 2 of
Chapter 64 of the North Carolina General Statutes. Further, if the CONTRACTOR utilizes a
subcontractor, the CONTRACTOR shall require the subcontractor to comply with the
requirements of Article 2 of Chapter 64 of the North Carolina General Statutes.
IRAN DIVESTMENT ACT
Provider hereby certifies that it is not on the North Carolina State Treasurer's list of persons
engaging in business activities in Iran, prepared pursuant to NCGS 147-86.58, nor will

Provider utilize on this agreement any subcontractor on such list.
DIVESTMENT FROM COMPANIES THAT BOYCOTT ISRAEL
Contractor hereby certifies that it is not on the North Carolina State Treasurer's list of
companies engaged in a boycott of Israel in violation of NCGS 147-86.80 et. seq. and that it
will not utilize on this agreement any subcontractor on said list.
Proposals must be submitted in the format outlined in this section.
Each proposal will be reviewed to determine if it is complete prior to actual evaluation. The County
reserves the right to eliminate from further consideration any response which does not follow the format
or is deemed nonresponsive; however, the County reserves the right to waive any irregularities or
formalities. NC General Statutes require that any proposal submitted include the Good Faith form
completed with original signatures included within this RFP as Attachment I. Any proposal which does
not include the signed Good Faith Form will be rejected, no exceptions can be made.
Each proposal shall include the following sections; each is to be appropriately tabbed and labeled and be
ordered as numbered below.
1. Project Cost
Expenses not specifically listed will not be considered reimbursable. Cost is to be a lump sum and
firm cost, not subject to increase.
2. Executive Summary - Proposals shall include a concise abstract stating the respondent's overview of
the project.
3. Move Contractor Qualifications Data
a. Company Profile - Provide general information on the responding company, including;
name, business address, local telephone number, officers of the Company, and contact
person for this project.
b. Project Team - Provide a list of the employees of the Company who will work on this
project.
c. References - Provide a minimum of five (5) references for move service projects in the last
thirty-six (36) months. Each reference shall describe the services and equipment provided,
project cost, and benefits to the owner. Provide the owner's name, address, telephone
number, and contact person for each reference. References for projects where the
responding firm was not the prime Contractor are not acceptable.
d. Litigation - Provide a description of any litigation to which your Company has been a party
in the last five years to the extent such litigation pertains to move services and installation
projects involving your firm.
e. Sample COI showing ability to meet minimum insurance requirements

4. Technical Approach

a. Proposed Scope of Work Project - methodology including technical approach and
understanding of the scope of the project. Proposals must indicate a clear understanding of
the scope of the work, including a detailed project plan for this project outlying major tasks
and responsibilities, time frames, and staff assigned for each category of the scope of work
identified above.
b. Provide details regarding the move services offered directly provided by the respondent,
and identify any related services required to be provided by others (including the County)
for full completion of this work. Proposals shall clearly distinguish the Move Contractors'
duties and responsibilities and those of the County. Absence of this distinction shall mean
the Move Contractor is assuming full responsibility for all tasks.
5. Project Time-Line
a. Proposals must provide chronological time-line of each task or event and estimated time
required to complete the engagement in an easy to read format. Note the start and end dates
included within the RFP.
6. Oversight from Single Contractor
a. The County strongly desires to contract with a single firm to accomplish all work and/or
services outlined in this Request for Proposal.
b. Any proposed sub-contractors must be identified in the Proposal response.
c. Any work not conducted by the Move Contractor or his sub-contractors must be disclosed.
d. Project Management - Indicate your firm's approach to managing the project. Include a
resume of the project manager responsible for the project.
