The County of Orange, (Health Care Agency) (hereinafter referred to as “County”), is soliciting proposals from qualified firms (hereinafter referred to as “Respondents”) for Correctional Health Discharge Medication Services . Respondents must meet the minimum qualifications and requirements as set forth within the RFP and be capable of providing services identified in the Model Contract, Attachment A (Scope of Work).
Discharge Medication Services. These services are essential for the implementation of California Advancing and Innovating Medi-Cal (CalAIM), aimed at creating a more coordinated, person-centered, and equitable health system that works for all Californians. Through the CalAIM initiative, Medi-Cal members have access to new and improved services to receive well-rounded care that goes beyond the doctor’s office or hospital and addresses all physical and mental health needs. The CalAIM Justice-Involved Initiative allows eligible Californians who are incarcerated to enroll in Medi-Cal and receive a targeted set of services in the 90 days before their release. This initiative aims to ensure continuity of health care coverage and services between the time they are incarcerated and when they are released. It also provides people who are reentering the community with the prescribed medications they need, and access to programs and services to support this important transition.
Through this solicitation, County aims to engage qualified, Medi-Cal enrolled pharmacies to fill a full supply of all active medications upon release for incarcerated individuals receiving pre-release services. Full supply is defined as the maximum amount that is medically appropriate and allowed by the Medi-Cal State Plan.
County intends to award a Contract for a term up to three (3) years in duration, renewable for two (2) additional one (1)-year periods.
