Overview

Summary

Provides ongoing service coordination to approximately 35 individuals per year in need of individualized, service linkage and coordination who are not eligible for other long-term care programs. Services focus on enhancing independence and vary in duration based on individual need. In addition, the program conducts a minimum of 12 outreach events per year, focused on improving community awareness of resources available for adults with hearing impairments and physical disabilities.
