NOTICE OF INTENT TO SOLE SOURCE

THIS IS NOT A REQUEST FOR QUOTE / PROPOSAL

The Defense Health Agency (DHA), Northeastern Healthcare Operations Contracting Division (NEHO-CD), on behalf of Walter Reed National Military Medical Medical Center, in support of the Medicine Department, Nephrology Clinic's requirement for a company that provides peritoneal dialysis (PD) kits and equipment to home End Stage Renal Disease (ESRD) dialysis patients. PD kits are typically furnished to patients based on a 28-day delivery schedule, which may result in thirteen or more annual shipments per patient, intends to award a firm fixed price contract on a sole source basis to Koffel Medical Supply, Inc, 1003 W. Park Avenue, Libertyville, Illinois 60048, CAGE: 5JGW0

Based on past performance and information obtained from the market place, the anticipated award will be made under the statutory authority permitting Other than full and Open Competition pursuant to FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements. The contract is intended to procure dialysis supplies and equipment with delivery to home PD ESRD dialysis patients.

This notice of intent is NOT a request for competitive proposals or quotations. Requests for copies of a solicitation in reponse to this notice will not be honored or acknowledged. Phone calls will not be accepted. A determination by the Government to not compete this proposed action based upon response to this notice is soley within discretion of the Government.
