ABOUT THE CITY OF ST. LOUIS HOMELESS SERVICES DIVISION
As administrator of federal, state and local funds, the City of St. Louis Department of Human Services,
Homeless Services Division (HSD) provides a comprehensive approach in responding to the diverse needs of
people experiencing homelessness or housing displacement in the City of St. Louis. HSD performs several
important functions to coordinate homeless services with the CoC in St. Louis City that includes the
following:
• Development, coordination, and monitoring of new and existing housing and services programs to
meet the needs of people experiencing homelessness and/or at-risk of homelessness,
• Negotiation of contracts with social services agencies ensuring that housing and services are
delivered efficiently, effectively and unduplicated,
• Collaboration and partnership with a broad spectrum of participants in the homeless services
delivery system such as health and human services professionals, advocates, government officials,
representatives from nonprofit agencies, businesses and persons with lived experience from the
metropolitan area, to address any needs that arise for the homeless community,
• Participation in a wide array of community meetings, committees, councils, training events and
national events and conferences, and community events, to educate and share information and
best practices in the field of serving and housing homeless individuals and families.
Through these activities, the HSD ensures an efficient homeless services system for funding the most
efficient and effective programs, reducing duplication of services and increasing innovative program design.
INTRODUCTION
The City of St. Louis is seeking a fair, equitable and transparent process to obtain a new sub-recipient for
two Continuum of Care Permanent Supportive Housing projects that were awarded through the
FY2024/FY2025 Continuum of Care NOFA competition to the City of St. Louis and subrecipient St. Patrick
Center. These projects were recommended for full renewal in the FY2025 Continuum of Care renewal
process. St. Patrick Center has given written notice to withdraw as sub-recipient for two of their Permanent
Supportive Housing projects. These programs fall under HUD’s Continuum of Care Program Grant and
applicants must meet all HUD subrecipient program requirements. Comprehensive information on the CoC

Applicants may respond to one or both projects. Because projects are renewals, applicants must adhere
to the current budget line-item allocations, grant term, number of households served (including current
project participants), and must continue to provide the essential housing and supports as detailed in the
original project descriptions as funded by HUD in the FY2024/FY2025 CoC renewal.

Current Project Implementation Description:
Project Live Permanent Supportive Housing Program first addresses an individual’s housing need and then
provides the ongoing, wrap-around support they needed to remain stably housed. The target population are
41 unaccompanied adults that meet the definition of chronic homelessness including individuals with
multiple morbidities such as mental health and substance abuse disabilities.
After 30 days of program enrollment, individuals work with a housing support specialist to identify and
select an apartment that best meets their housing needs, including characteristics such as location (i.e.,
proximity to transportation, family, etc.) and accessibility. The housing support specialist will also assist
participants in addressing any housing barriers; ensure they understand the expectations within their lease
agreements; and communicate with landlords in regard to specific issues related to the rental property
and/or any issues that may arise while the consumer resides there. Participants receive a move-in kit that
consists of household items often needed to become established in their new residences, and may be
referred to a collaborative partner for furniture needs. Each individual also receives immediate financial
assistance with rent and utility deposits and ongoing rent and utility assistance while enrolled in the
program. Once participants have a stable source of income, they will be responsible for contributing a
portion of their monthly income towards rent and utilities.
Participants begin case management within 30 days of program enrollment, which helps them to become
integrated in their new communities and set and begin working towards goals related to sobriety, stability,
and self-sufficiency. Through intensive case management, participants are connected to existing community
services as a long-term goal for eventual transition out of permanent supportive housing support into
independent and stable housing. These services may include mental health counseling, substance abuse
counseling, financial stability counseling, health counseling, and healthcare liaison activities. Although the

frequency of in-home case management will be based on individual need of each participant, visits will
occur weekly at a minimum.

Current Project Implementation Description:
Rosati Center is a Permanent Supportive Housing (PSH) program that offers intensive case management
(ICM) and tools for daily independent living for homeless individuals. The target population is
unaccompanied men or women who meet the HUD definition of chronically homeless; are 18 years of age
or older; and who experience persistent mental health and/or substance abuse issues. Most of the
participants receive SSI and/or SSDI as a primary source of income.
Rosati residents receive in-home assistance from a multi-disciplinary team including a manager, nurse,
substance abuse counselor, mental health counselor and financial counselor. In-home visits occur monthly
at a minimum, but the frequency of visits is based on the needs and active symptoms of each resident. The
team is on call 24 hours per day, and assists each resident in developing and achieving goals related to
sobriety, stability and self-sufficiency.
The program also encourages participants to improve their quality of life by engaging in social activities and
volunteer opportunities that provide a greater connection to the community. Individuals who qualify for
housing placement at Rosati Center are housed either in the 12-unit Rosati Apartments or in a scattered site
apartment in the City of St. Louis. Housing placement in an on-site or off-site unit depends on each
resident’s level of need and ability to live independently. For either placement type, however, case
managers and residents work together to develop individualized treatment plans based on an initial
assessment that evaluates the person’s physical and mental health, drug and alcohol use, and financial
stability. Case managers and counselors then conduct regular in-home visits with residents to assess
