RFA-RC-2025-002 2026 Application for Nursing Assessments for Medical Assistance (Medicaid) Personal Care Services Recipients

The Rockland County Department of Social Services (DSS) is accepting applications from public, not-for-profit, or proprietary agencies wishing to provide nursing assessments for Medical Assistance (Medicaid) Personal Care services recipients in Rockland County for the period January 1, 2026 - December 31, 2026. An applicant may be a home health agency, voluntary or proprietary agency that is legally qualified under New York State law to enter into a contractual relationship with a social services district and that is certified by the New York State Department of Health (NYSDOH) to provide nursing assessments, per 18 NYCRR Section 505.14.
