The Wisconsin Department of Health Services’ (DHS) Maternal and Child Health (MCH) Title V block grant provides funding and support to local and Tribal health agencies and other organizations to improve the health and supporting the well-being of mothers, infants, children, and families.
The purpose of this funding opportunity is to provide information and referral services to children and youth with special health care needs (CYSHCN), their families, and providers.
All funding application submissions are due on August 13, 2025, by 11:59 p.m. CDT.
Applicants will be notified of award decisions no later than September 5, 2025.
