The Wisconsin Department of Health Services’ Maternal and Child Health (MCH) Title V block grant provides funding and support to local and Tribal health agencies and other organizations to improve the health and supporting the well-being of mothers, infants, children, and families.
The purpose of this funding opportunity is to improve medical home best practices for children and youth with special health care needs (CYSHCN) and increasing access to a usual source of sick care for all children.
All funding application submissions are due on August 13, 2025, by 11:59 p.m. CDT.
Applicants will be notified of award decisions no later than September 5, 2025.
