The Wisconsin Department of Health Services’ Maternal and Child Health (MCH) Title V block grant provides funding and support to local and Tribal health agencies and other organizations to improve the health and supporting the well-being of mothers, infants, children, and families.

The purpose of this funding opportunity is to strengthen Wisconsin’s statewide infrastructure that supports Food is Medicine coordination and implementation to improve nutrition security for all families.

All funding application submissions are due on August 13, 2025, by 11:59 p.m. CDT.

Applicants will be notified of award decisions no later than September 5, 2025.
