2025-2030 Physical Therapy Services in Franklin County

OPWDD's Contract Management Unit for Sunmount DDSOO is soliciting sealed bids from a responsible and qualified Physical Therapist to provide physical therapy services to people with developmental disabilities residing in our state operated/certified facilities located in Franklin County. Services are to be provided at Sunmount DDSOO’s Campus at 2445 State Route 30, Tupper Lake, NY 12986, as well as adjoining locations at 380 Hosley Ave., Tupper Lake, NY 12986 and 390 Hosley Ave., Tupper Lake, NY 12986. OPWDD is not looking for temporary services. OPWDD is looking for a qualified Physical Therapist who will commit to the full, five-year term of the contract.

Qualifications:

Contractor must submit, with the bid, the Physical Therapist’s professional license(s), résumé, and references demonstrating they meet the qualifications listed below. A Physical Therapist duly qualified, licensed to practice and registered in New York State. A Physical Therapist with at least one year of experience providing services to people with developmental disabilities.

Services to be provided include but are not limited to:

Provide physical therapy services under the supervision of the Head Physical Therapist or designee. Provide appropriate documentation of an individual’s physical therapy treatment plan and progress.

The Physical Therapist will provide OPWDD with copies of professional licenses and registrations and upon request will provide verification of all qualifications and a copy of their New York State Driver's license.

Prevailing Wage does not apply to this contract.

OPWDD will select the responsible and responsive Bidder that will provide the lowest Total Annual Estimated Cost for Physical Therapy Services.

Only proposals judged to be responsive to the submission requirements set forth in this IFB will be evaluated. Any alteration of the cost proposal form may result in disqualification of the bid. OPWDD has the right to reject any and all offers.
