SUBMITTAL - Scope of Work, Response

The offer must provide the following:

Provide proof that you are a manufacturer, authorized dealer/reseller, or supplier of the products offered.

Provide fulltime, permanent business address, email and phone number

Affirm that you have manufacturing and distribution centers that maintain a minimum inventory of nine thousand (9,000) gallons dedicated to each distribution location at all times.

Provide the address of the primary distribution facility located within five hundred (500) miles of the City of Austin, the address of the alternate distribution facility, and the available storage volume dedicated to this contract at each facility.

Affirm that you will provide alum that is not repackaged.

Provide NSF approval of shipper (either manufacturer or shipping company)

Provide the manufacturer's name and the manufacturer's shipping address/location.

Provide three (3) current municipal wastewater references within the State of Texas that utilize the offered product for alum treatment at their wastewater treatment facilities. Each reference shall include the facility name and address, contact person, phone number, and the current application of the product. References must be included with the offer or provided within three (3) business days of request by the City. See Submittal-References

Provide a current Federal Department of Transportation (USDOT) number for the Contractor or, if subcontracting, for the trucking company.

Provide a certified Certificate of Analysis (COA) that meets the applicable requirements of this specification for alum, along with a Safety Data Sheet (SDS).

Provide an affidavit stating that the Liquid Alum complies with the requirements of this specification and the most recent edition of AWWA B403.
