(Patriot) is requesting proposals for the Building Demolition of selected structures throughout Greene County, Ohio. Generally, this work will consist of demolition and permanent removal of existing structures at the property locations identified in this Request for Proposal (RFP). This work is being performed under a grant agreement between the Greene County Department of Development and the Ohio Department of Development (ODOD) under the Building Demolition and Site Revitalization Program. Detailed information concerning the RFP, including the scope of work, due date for submittal, and prebid meeting, submittal procedures, requests for additional information, etc., is provided in the enclosed form.

Special Requirements:
Asbestos Testing and Universal Waste Surveys have been performed for each property, and a report is included with this RFP as Attachment D. Contractor
is responsible for OEPA Notification and to have asbestos abatement performed by a licensed company and universal waste disposal in accordance with applicable regulations. The Ohio Department of Development Asbestos Compliance Form, included as Attachment E, is to be completed and returned with project close-out documentation. Costs for these items should be included in the project quote. A 10-working-day notification to OEPA is required from the contractor before demolition of any commercial or residential property begins (unless emergency demolition). All work which the Contractor is required to perform shall commence within 30 working days after the OEPA ten-day notification expires and shall be completed by the date specified by Greene County Department of Development on the NTP. In no case shall project completion extend past 75 calendar days, unless previous arrangements have been made and agreed upon in writing by Greene County Department of Development. In addition, local permits and notification are required as per applicable County and/or local ordinance. Contractor must provide at least 48 hours' advance notification to Patriot Engineering and Environmental (Patriot) to allow for scheduling of contractor oversight. At a minimum, Patriot will complete, in conjunction with the contractor, the Pre-Demolition (this is required to be completed by a Patriot representative before site demolition activities commence) and Post-Demolition (this is required to be completed by a Patriot representative and signed by the demolition contractor upon completion of work, prior to demobilization of contractor equipment) Site Checklists. Examples of both are included in Attachment F.

Bids shall be submitted in sealed envelopes, properly identified with the Bidder's name, contact
number, bid opening date, and a statement that it is a sealed bid covering the materials or services
for which the bid is submitted. Bids shall be addressed to:

A mandatory pre-bid meeting will be held at the site on Monday, March 16th at 1:00 p.m.
EDT. Parking at the site is very limited and bidders should park along the driveway at 3341 Jasper
Road (see parking map included). Proposals should be submitted on the attached Greene County
Department of Development Proposal Form. Fill in all the requested information. Asbestos
abatement and universal waste removal is to be included. Provisions contained in Attachment
A, Greene County Department of Development Demolition Project Specifications, apply to this
work.
Greene County Commissioners reserve the right to reject any and all bids, to waive any and all
informalities, and to disregard all non-conforming, non-responsive, or conditional bids. The
basis for Contract award shall be based on a Best Value Selection for this project. As such,
Greene County intends to use a selection process for services where total cost, as well as other
non-cost factors, are considered in the evaluation, selection, and final award of this contract.
Greene County will consider the below factors to help determine the best value. Each proposal
will be scored on a points system, with the bidder achieving the highest number of points, out of
a maximum of 25 points total, being selected as the winning bidder.
i. Total Cost (0-10 points). Total cost will be evaluated for each bidder. The bidder with
the lowest cost will be assigned 10 points, while the next lowest bidder will receive 9
points, third lowest bidder awarded 8 points, etc.
ii. Past Performance and the qualifications and experience of the Bidder, specifically
with prior projects executed with the Greene County Department of Development (0 –
5 points).
iii. Bidder maintains an office within Brown County, Ohio (0 – 1 point).
iv. Compliance of Bid Response with the prescribed requirements of this Bid Package (0
– 4 points).
v. Bidder’s proposed schedule for completion of tasks specified in this Bid Package (0 –
5 points).
A purchase order or a Notice of Award and Authorization to Proceed letter (example included as
Attachment B) will be issued for each demolition project. The selected bidder will be required to
return a signed copy of the “Notice of Award and Authorization to Proceed” letter, along with the
completed Proposed Project Schedule.
The Contractor shall submit the Contract Completion Certificate and Pay Application for the
Demolition of Residential Structures Affidavit, included as Attachment C, along with the
appropriate invoice, manifest/waste shipment records (ACM/C&DD) of disposal, permits and
notifications, and photos. Copies of all disposal documentation shall be provided with the payment
application as instructed. As this work is being completed with ODOD grant funding, Contactors
shall only submit one (1) invoice per month. Greene County will pay invoices within 10 business
days of receiving reimbursement funds from the State.

Special Notes: Demolish and remove 2-story residential home and associated foundation,
including basement walls. Backfill basement. Properly abandon well on property. Remove
surrounding trash around residential structure. Remove and properly dispose of universal
waste. Limited tree removal will be required and discussed during pre-bid meeting. Grade,
seed/straw disturbed areas.
ASBESTOS: Results of the sampling indicate that transite siding, joint compound and
textured drywall, contain asbestos percentages greater than 1%- see report.
Abatement required before demolition begins.

Photographic evidence shall be submitted of the property before, during, and after demolition
work is completed.
In the event borrow source material is required to be imported from an off-site source(s), the
Environmental Questionnaire for Proposed Borrow Source form, included as Attachment F, must
be completed and submitted by email to Patriot (ecorbin@patrioteng.com and
mweinstein@patrioteng.com) for approval prior to bringing borrow material on-site. Before
property is backfilled Photographic evidence shall be submitted of empty basement voids.
BOND REQUIREMENTS
A BID BOND is not required to be submitted for this proposal.
In lieu of a PERFORMANCE BOND, the successful BIDDER will be required to enter into a
Retainage Agreement with Patriot as an assurance for timely completion of the entire project in a
quality manner in accordance with the CONTRACT DOCUMENTS. Implementation of this
Retainage Agreement eliminates the requirements for a BID BOND, PERFORMANCE BOND,
and PAYMENT BOND. The schedule of this Retainage Agreement will be structured as follows:
1.
Retainage Amount: Eight percent (8%) of each monthly progress payment request
will be withheld from payment.
2. Maximum Retainage Amount: When fifty percent (50%) of the project is completed
(as determined by 50% of total project costs being billed via monthly progress
payment requests), no additional retainage will be withheld.
3. Retainage Release: Within thirty (30) days of the project acceptance all retainages
withheld will be released to the Contractor.

Required insurance is detailed in Item #3 under the General Conditions of the Contract section of
this RFP.
