Description:

PSA26 Area Agency on Aging (AAA) is seeking proposals from interested and qualified organizations and firms to administer the CalFresh Healthy Living (CFHL) program which provides evidence-based nutrition education and obesity prevention services based on United States Department of Agriculture, Food and Nutrition Services guidance. AAA and their contracted providers offer CFHL interventions to 60+ older adults at or below 200% of the Federal Poverty Level, at eligible sites including Title IIIC congregate nutrition sites and other venues meeting eligibility criteria. In addition to direct education, the CFHL program incorporates indirect education and Policy, Systems, and Environmental (PSE) change approaches to deliver effective, evidence-based nutrition education and obesity prevention programs. Completed proposal packages must be received at the PSA26 AAA office at 16170 Main St Unit F, Lower Lake, Ca 95457, by the deadline of August 11, 2025 no later than 3:00pm. Please note that all costs incurred in the preparation of the proposal for this RFP are the responsibility of the applicant and are not subject to reimbursement by the PSA26 AAA.

Qualifications:
Eligible Applicants: The applicant organization must be a 501(c) (3) non-profit corporation or government agency. Other for-profit agencies or business can apply as well. All potential contracts with for-profit businesses must be approved by the State of California Department of Aging. Individual providers (sole proprietors) will be considered, and applicants are expected to possess a minimum of two years’ experience in the services for which they apply for under the RFP.
Special Requirements:
Expectations of the Program: ? Implement evidence-based direct nutrition education as well as either indirect nutrition education and/or a PSE change approach at approved sites. ? Collect demographic data and surveys (provided) from participants. ? Import data into CFHL data collection website. ? Collaborate with other CFHL Implementing Agencies in your country to complete an Integrated Work Plan. ? Participate in your local Country Nutrition Action Plan. ? Attend quarterly calls with CDA and complete a quarterly report. ? Maintain an approved budget. ? Attend training and other development activities as needed.
Miscellaneous:
Letter of Intent (LOI): A letter of intent is required of all vendors who intend on submitting a bid in response to this RFP. The letter of intent is due to PSA26 AAA by July 21, 2025. If a letter of intent is not submitted by the required deadline the bid/proposal will not be accepted.

Submission Requirements: Four (4) paper copies and one (1) electronic copy of the proposal will be received by the Area Agency on Aging of Lake and Mendocino Counties (PSA26 AAA), 16170 Main St Unit F, Lower Lake, Ca 95457, aaa@lakecountyca.gov.
