25-P-01TF, Group Vision Insurance

The purpose of this Request for Proposal (RFP) is to solicit competitive sealed proposals for a Contract for Group Vision Insurance. Volusia County Government (County) is seeking proposals for voluntary, fully insured group vision insurance for County employees, retirees, eligible dependents (including legally married spouse, children up to twenty-six (26) years of age, over-age dependents twenty-seven through thirty (27-30) years of age) and COBRA participants.

Volusia County Government (County) is seeking proposals for voluntary, fully insured group vision insurance to County employees, retirees, eligible dependents (including legally married spouse, children up to 26 years of age, overage dependents 27-30 years of age) and COBRA participants.

Premiums will be one hundred (100%) percent contributory under the County’s section 125 cafeteria plan, and eligible employee/former employee would be allowed to enroll/terminate group vision insurance during the annual enrollment period or during a qualifying life event status change.

Employee paid premiums are payroll deducted. There are twenty-six (26) payroll periods, annually.

The County outsources COBRA and retiree participants' enrollment and billing administration to a third-party administrator, currently Chard Snyder.

Current plan documents are provided in Exhibit E - Plan Document. The benefit includes exams, frames, and lenses.
