The Health Department is seeking a contractor to conduct a community health needs assessment and community health improvement plan and develop a written report(s) of the findings. The community health needs assessment and community health improvement plan must align with the Public Health Accreditation Board’s Standards and Measures for Initial Accreditation, Version 2022.
The New Haven Health Department (NHV Health) is pursuing its initial accreditation through the Public Health Accreditation Board . A Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) are two required documents for accreditation.
The CHA will provide critical information about the health of New Haven residents and suggest ways to improve the health and ensure equity of all New Haven residents. The CHA will be used to set priorities, including policy, funding allocations, and program development. The CHA should include information on mortality and morbidity, quality of life, attitudes about health behavior, socioeconomic factors, environmental factors (including the built environment), social determinants of health, community narrative, assets, and stories. Data must be obtained from a variety of sources, using various data collection methods.
The CHIP is a long-term, systematic plan to address issues identified in the community health assessment. The purpose of the community health improvement plan is to describe how the health department and the community it serves will work together to improve population health in the jurisdiction. The plan reflects the results of the CHA.
Total funding available is $60,000.
