This solicitation is issued as a Request for Quote (RFQ) from the USPFO for New York, Purchasing and Contracting Division, Latham, New York. The solicitation number is W912PQ-26-Q-A004 for a firm-fixed-price services purchase order for medical surveillance examination services as defined in the Performance Work Statement (PWS) under NAICS 621511. Principle NAICS is 621511 and size standard is $41,500,000. This acquisition is utilizing procedures in FAR part 12. This requirement is set aside 100% for Small Businesses. Quotes are due 16 JAN 2026.
The Contract will be awarded from this solicitation to the responsible offeror with the best priced quote that also conforms to the requirements of the Performance Work Statement. Each factor will be evaluated as acceptable or unacceptable. The following shall be used to evaluate offers: 1 - Provide a capability statement that includes company’s small business status; CAGE/UEI (must be 100% small business set aside); company’s core competency; and when the quote expires. 2 - Provide evidence of license/certifications. License/certification documentation shall show individual/company identification and date certified.
The agency will first evaluate price, and then evaluate the apparent lowest-price proposal for acceptability under the two factors listed above. All two factors above shall be considered acceptable for award to be made. If the lowest price offeror is evaluated as unacceptable, the agency will then consider the acceptability of the next lowest price offer, continuing this process as necessary. Award will be made without discussions. The Government will not accept partial quotes/offers.
In order to be eligible for award, the successful offeror's System for Award Management (SAM) (https://www.sam.gov/) registration must be active and include representations and certifications.
