Statement of Work
I. SCOPE OF WORK:
The Pennsylvania Department of Corrections, CI Fayette, requires the purchase,
delivery and installation of a license plate blanking line.
Delivery will be made to the CI warehouse at SCI Fayette located at 50 Overlook
Drive, LaBelle, PA 15450.
All bids must be submitted electronically.
II. ISSUIING OFFICER
Questions regarding the bidding or contracting procedures should be directed
towards Amanda Wasko at amawasko@pa.gov.
An optional site visit inspection will be held on June 2nd at 9 am. Prospective
bidders must email Jake Laskody (jlaskody@pa.gov) the completed Clearance
Check Request Form no later than May 19th. More information about the
Pennsylvania DOC clearance certificate process can be found at
https://www.cor.pa.gov/Facilities/Pages/Clearance-Reguest.aspx. Questions
must be submitted electronically to amawasko@pa.gov for this event. Any
discussions or questions raised on site visits are not officially binding unless
submitted to amawasko@pa.gov and responded to by the Commonwealth on
eMarketplace.
III. CONTRACT REQUIREMENTS
The contractor shall provide:
• Blanking Line
• Delivery
• Installation/Startup
• Maintenance Contract
The blanking line must meet the following specifications:
• Horizontal aluminum decoiler. Able to support 2 rolls of aluminum coil (minimum of
4,000 lbs.)
• Metal straightener – Must accommodate 7 1/8” up to 12 1/8” wide material x .028
thick.
• Aluminum heating unit – Must heat aluminum to minimum of 80 degrees F.
• Sheeting applicator – Must be able to apply 7 1/8” up to 12 1/8” wide reflective
sheeting. Needs to include automatic stretch control.

• Mechanical blanking press – Needs to be capable of high-volume production. Must
include 2 sets of blanking dies with slug collector. Electrical requirements are 480-
volt 3 phase.
• Qty (2) 12 1/8’ blanking die set.
• Qty (2) 7 1/8” blanking die set.
Bidders must upload a complete identification in its Bid of the product it is offering
by trade name, brand and/or model number as well as any descriptive literature.
Any uploaded documents are for informational purposes only. The DOC will not
agree to alternate payment terms or additional terms and conditions included in any
informational documentation beyond those listed in Attachment B – Terms and
Conditions.
For delivery, the contractor shall provide all rigging or other services as required to
off load, spot and secure the equipment in the appropriate location. The machine
shall by in a “ready-to-run” state. All delivery and freight charges must be included
within the quoted price for the delivery line item. The facility has a loading dock.
Start-up costs include equipment startup, installation, testing and training for 12
people. CI Fayette shall provide all electrical connections.
The maintenance and support contract will cover one year. CI Fayette may renew
the maintenance and support contract in one-year increments for a total of five
years. Pricing of the maintenance contract will remain firm across the initial year and
the four optional renewal years. The maintenance and support contract includes,
but is not limited to, yearly preventative maintenance visits, remote diagnostics and
on-site emergency services.
IV. CONTRACTOR REQUIREMENTS
Pursuant to Executive Order 2021-06, Worker Protection and Investment (October
21, 2021), the Commonwealth is responsible for ensuring that every Pennsylvania
worker has a safe and healthy work environment and the protections afforded them
through labor laws. To that end, contractors and grantees of the Commonwealth
must certify that they follow all applicable Pennsylvania state labor and workforce
safety laws. Such certification shall be made through the Worker Protections and
Investment Certification Form (BOP-2201) and the completed form must be
submitted with the bid, proposal or quote.
All vendors or their subcontractors, who utilize regular delivery transportation, will
have to submit clearance checks through the Original Vendor, for all drivers
(regardless if delivery is inside or outside the fence). Common carriers will not be

required to render clearance checks, however are subject to search and escort
procedures. ALL CARRIERS reporting to the outside warehouse must secure their
vehicles (windows closed and doors Locked) sign in at the warehouse and wait for
further directions / instructions from the Correctional Staff. Delivery Truck must
leave the institution empty
V. BID AWARD
It shall be understood and agreed that any quantities listed in the solicitation are
estimated only and may be increased or decreased in accordance with the actual
requirements of the Department of Corrections.
Unless otherwise indicated, the unit price must include all labor, materials,
equipment, tools, insurance, delivery fees and all items necessary for the completion
of the project. In cases of discrepancies in prices, the unit price will be binding unless
the unit price is obviously in error and the extended price is obviously correct, in
which case the erroneous unit price will be corrected.
VI. BID Results
Bids will be opened on the date and time specified in the invitation for bid. Bid
tabulations will be posted on the day after the bid opening, on the Department of
General Services’ eMarketplace website http://www.emarketplace.state.pa.us/).
Tabulations are for information only and do not constitute actual award or
execution of a contract. The results of the apparent bidders and all bids are under
review until final award of the purchase order.
