NOTICE OF INTENT TO AWARD SOLE SOURCE. The Network Contracting Office (NCO) 16, located at Alexandria VA Healthcare System, 2495 Shreveport Highway, Pineville, LA 71360 in support of Michael E. DeBakey VA Medical Center (MEDVAMC), intends to solicit and award Sole Source, Firm-Fixed-Price, Purchase Order to a Small Business source, Armamentarium, Inc, 22317 Goslin Road, Spring, TX 77389. The Michael E. DeBakey VA Medical Center, 2002 Holcombe Blvd, Houston, TX 77030 has a requirement for a contractor to provide all shipping and delivery of Name Brand TeDan Phantom LS Front-Load Posterior Lumbar Surgical Access System.
The North American Industry Classification (NAICS) code for this acquisition is 339112, Surgical and Medical Instrument Manufacturing, and the Small Business Administrative Size Standard is 1,000 employees.
