The purpose of this RFP:The Utah Department of Health and Human Services (DHHS) is the single state agency responsible for the administration of the Utah Medicaid program. The Department has several offices and divisions, including the Division of Integrated Healthcare (DIH) which oversees the Utah Medicaid program.
The DIH seeks up to two entities to provide temporary medical respite to individuals experiencing homelessness or are at risk of homelessness as defined by 24 CFR 91.5 covered under the Adult Expansion Medicaid program and Targeted Adult Medicaid (TAM), in accordance with an 1115 Waiver as approved by the Centers for Medicare and Medicaid Services (CMS). Use of subcontractors is allowable.
The duration of the contract with two entities will be five years.
It is anticipated that this RFP will result in multiple contract awards.
The methodology or formula that will be used to determine the number of contract awards is: It is anticipated that this RFP will result in up to two contract awards. The methodology or formula that will be used to determine the number of contract awards is: The two highest scoring vendor(s), given there are at least two vendors that meet the minimum threshold, that receive a score of at least 70% of all technical points, will receive a contract. There are 250 technical points available and 27.8 cost points. Thus, the two highest scoring vendor(s) that achieve a minimum score of 175 out of 250 technical points will receive a contract.
Length of Contract: The duration of the contract with two entities will be five years.
Vendor must guarantee its pricing for at least one year.
