Quoted price must valid for 60 days.

Bidders that hold an active/current DAS State Contract are required to attach a proposal with the following information referenced below.

Quantity and all costs with breakdown of UOM
Period of Performance
DAS State Contract #

Failure to attach a quote with all the requirements listed may result in disqualification of award.

Selection will be based on the lowest responsive and responsible quote.
