Heartland Whole Health Institute is now accepting Requests for Information from implementation partners for a large-scale health care initiative designed to transform how chronic cardiometabolic diseases are prevented, managed, and reversed.
Organizations and vendors that offer the following services are encouraged to respond: smoking cessation programs, and peer-led interventions.
