The purpose of the grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost.The purpose of this subrecipient grant opportunity is to support procurement, implementation, and ongoing management, maintenance, and operations services for a digital closed-loop referral platform to strengthen communication and collaboration among health care, mental health, substance-use-disorder, and social service providers. The system should ensure patients successfully connect to needed community supports, such as housing, food assistance, and transportation. By enabling real-time data sharing and tracking of referral outcomes, the platform should reduce duplication of services, improve accountability, and promote cost efficiency while ensuring more seamless, person-centered care across Vermont’s health and social service systems. The State of Vermont seeks a qualified third-party organization (“Grant Recipient”) to serve as the program manager and implementation lead for the Shared Services – Closed-Loop Referral System activity under Vermont’s Rural Health Transformation (RHT) Program.
