Project Description: The City of Kansas City, Missouri (“City”), through the Health Department (HD), is seeking proposals from qualified community-based organizations that serve targeted communities and populations where chronic disease rates are highest, to implement initiatives that support community members in altering their behaviors to lower the risks of chronic disease.
