Kayenta Health Center: Non-Personal Service Contract - Pediatric Physician Services

The Indian Health Service, Kayenta Health Center, located in/at Highway 160, Mile Post 394.3 has an upcoming requirement for one (1) non-personal service contract for one (1) Pediatric Physician.

The Period of Performance is a base period plus one (1) option period (six (6) months per period):

Base Period: 11/01/2025 through 04/30/2026

1st Option Period: 05/01/2026 through 10/31/2026

The North American Industry Classification System or NAICS code is 621111 – Offices of Physicians with a small business size standard of $16.0 Million.

The Request for Quotations No. 75H71025Q00030 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Acquisition Procedures will be issued on or by September 12, 2025.

Evaluation factors may include:

• Qualifications of the Vendor, technical capability, and approach in order to meet the government’s requirement as defined in the Performance Work Statement (PWS).


• Experience (Pediatric Medicine Key Personnel)
  
  
  
  • Key Personnel’s Licenses/Certifications – BLS, ACLS, PALS, Current Medical License, and DEA License.

Quotes will be due on or by September 22, 2025. The acquisition is a 100% Indian Small Business Economic Enterprise (ISBEE) set aside.
