Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71325Q00039 has been issued as a Request for Quotation (RFQ) for a firm fixed price commercial supply contract for twenty-five (25) brand name KaVo Cellular Optic INTRAflex LUX 2 Air Motor System 2325 LN slow speed dental handpieces (Part no. 0535.6210), and twenty-five (25) brand name KaVo 460LED MULTIflex LUX couplers (Part no. 1007.3201). The delivery date is within 60 days after receipt of order. The delivery location and place of acceptance is Chemawa Indian Health Center, Salem, Oregon, 97305, F.O.B. Destination.





The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-04 dated June 11, 2025. QUOTATION INSTRUCTIONS:





(a) The basis for award is lowest price technically acceptable (LPTA). Award will be made to the responsible offeror whose quote conforming to the solicitation is made at the lowest price. Each offeror will submit a completed copy of ATTACHMENT 1 - SCHEDULE OF ITEMS.





(b) IHS IEE Representation form. Each offeror will submit a completed copy of ATTACHMENT 2 - IHS IEE REPRESENTATION FORM.





(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.





(d) Award will be made to the responsible respondent pursuant to FAR subpart 9.1. Quotes are due by
