STATEMENT OF WORK

1. Introduction/Background

The Central Texas Veterans Health Care System (CTVHCS) Surgical Service – Ophthalmology Section requires the purchase of Marco EZ Tilt Chair Warm Grey - 1264 Upholstery Color- Includes EZ Tilt Cushion-Charcoal and Chair Installation to replace end of life equipment. The requested items are needed for patient care eye exams. These chairs make reclining any size patient easy and instantaneous no electronic motors to slow you down or patient care examinations.

These items are available for purchase on GSA/FSS Contract number 36F79722D0203.

Objectives

To obtain the following items for the Ophthalmology Section of Surgical Service:

• Marco EZ Tilt Chair, Warm Grey - 1264 Upholstery Color


• EZ Tilt Cushion-Charcoal


• Chair Installation

All items must be covered by manufacturer’s warranty and procured through a manufacturer approved distribution channel. Sellers must be able to document their document ability to provide items through manufacturer approved distribution channels upon request.

Scope of Work

The vendor will deliver the following items to include installation and on-site training to CTVHCS located at Temple Ophthalmology Section 1901 Veterans Memorial Drive Temple, TX 76504
