REQUEST FOR INFORMATION / SOURCES SOUGHT for BD Alaris Injection Pumps and Modules

DEPARTMENT OF VETERANS AFFAIRS, VISN 23 Source Sought: The VA is seeking business sources qualified to provide some, or all, of the items listed below as Brand Name or Equal. This is NOT a solicitation.

POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE:

1. Company name, address, phone number, primary contact(s), e-mail address.

2. State if your firm distributes the Brand Name items, or Equal items.

3. SAM UEI number.

4. NAICS (North American Industry Classification System) code 339112 that your firm uses for injection pumps and modules.

5. State if your firm is a large business, small business, service-disabled-veteran-owned
small business, veteran-owned small business, HUB zone 8(a), women-owned small business or a small disadvantaged business.

6. State if your firm has the required items on Schedule (FSS, GSA, SEWP, other ).

7. Provide a letter from the manufacturer verifying your firm is an authorized distributor.

8. Provide the country of origin for each of the required items.

REQUIRED ITEM and Salient Characteristics:
Contractor shall provide BD brand-name only Alaris equipment, software package, accessories, licenses, WIFI capabilities, and installation services as follows:
BD Alaris PC Unit, Model 8015 / 8015 BD 150 QTY
BD Alaris Pump Module, model 8100 / 8100 BD 224 QTY
Alaris PCA Module / 8120 20 QTY
BD Alaris EtCO2 Module / 8100 BD 20 QTY
Equipment Check-in Services
Channels compatible with the Alaris large volume infusion pump PC units 8015, that are already on site
Allows for continuous and intermittent deliver capabilities for various fluids
Must have alarm, infuse, and standby notification
Must have rate (mL/h) reading
Must be able to select channel, pause, turn channel off, and restart pump
Must be compatible with current Carefusion/Alaris administration sets and consumables
Must include Guardrails point-of-care software for each module to help reduce medication errors, track and measure system performance
