OFFERORS READ THIS INFORMATION FIRST.
This a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is a solicitation conducted using FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested.
Solicitation Number: 36C26323Q0413
The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-01 dated 12/30/2022.
This procurement is 100% set-aside for Small Business entities. All offerors shall have an active System for Award Management (SAM) registration at the time of offer and prior to contract award.
The VA anticipates the award of a Firm Fixed Price (FFP) contract for Calibration of Audiology Equipment Services to support the St. Cloud VA Health Care System (VAHCS). See Statement of Work and Price/Cost Schedule for specific requirements within the attached RFQ SF1449 document.
The period of performance is from April 16, 2023 to April 15, 2024 and includes four (4) one-year option periods to be exercised at the discretion of the Government.
Solicitation Questions. All solicitation questions shall be submitted in writing via email to Joseph.Bennett@va.gov. Questions must be received by 12:00 PM CST, March 30, 2023.
Submission of Offer: Refer to FAR Provision 52.212-1, and Addendum to FAR 52.212-1 within the attached solicitation document for instructions on submitting an offer and to determine the required documentation to be submitted with their offer.
Evaluation Factors. Best Value. Offerors shall submit required documentation outlined in FAR Contract Clause 52.212-2, Evaluation Factors. Offerors are responsible to review the solicitation s FAR provision 52.212-2 to determine the required documentation to be submitted with their offer.
Offer Due Date: Quotes are due to the Contracting Office by 04:00 PM CST March 31, 2023. Quotes shall be emailed to Joseph.Bennett@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Officer prior to the solicitation closing date and time.
