Bid Attributes
1 INTRODUCTION
The City of Hot Springs, Arkansas, is seeking proposals from certified Contractor for pest control services as a Term
Contract (TC). This Request for Proposals (RFP) contains all the information necessary to prepare and submit a
proposal. Prospective Respondents are advised to study this RFP completely and follow the instructions exactly.

3 SCOPE OF SERVICE
The following scope of service describes the desired requirements for this service. The Contractor shall be licensed
to engage in pest control service work. The Contractor shall furnish all labor, supervision, transportation, tools,
materials, and supplies necessary to perform pest control services in accordance with the guidelines established by
Federal, State, and Local Government laws, rules, and regulations. Inspecting and controlling all vermin, such as,
but not restricted to: fleas, stinkbugs, ants, wasps, bees, mice, roaches, etc., at the locations. Services for rodent
control will be on an as needed basis. Respondents shall price the rodent control separately. Complete the "Bid
Line items" on Line Item tab.
The Contractor shall provide service to the locations in the frequency shown and as necessary to control pests.
Services for the buses can be done during normal work hours, or before 10 AM CT on Saturdays and Sundays or
treat the buses after 8 PM CT. The awarded Contractor is required to contact the Intercity Transit Department in
advance to schedule. Service to be performed by the Contractor that is required more frequently than is shown in
the RFP will be at no additional cost to the City. In other words, the Contractor is required to provide service as
often as needed to adequately control pests for the amount of their proposal.
The Contractor shall obtain the signature of a City employee, preferably a supervisor or office manager when
service is performed at a City location. Monthly invoices are to be submitted to the City. Contractor's employees who
perform service at City locations are to wear or display identification that they represent the Contractor.

The acquisition will use a grant from the Federal Transit Administration (FTA), and the FTA Third-Party clauses will
be included in the contract. The City's preference would be to award the RFP to one (1) Contractor for all location,
but reserves the right to award by location. The contract resulting from this RFP will be for one (1) year with five (5)
twelve (12) month terms, subject to annual budget approval by the Hot Springs Board of Directors. Contract will
automatically renew annually unless either party supplies thirty (30) day written Notice of Cancellation. Any
escalation in price shall be subject to paragraph 16 of the Terms and Conditions - RFP. City reserves the right to
accept or decline any and all price increases.

4 FEDERAL AWARD IDENTIFICATION - SYSTEM FOR AWARD MANAGEMENT (SAM)
In order to be eligible for a contract award assisted by the Federal Transit Administration (FTA), all prime
contractors must be registered to do business with the U.S. Government.
Prior to any informal contract commitment and published notice of intent to award by the City of Hot Springs, eligible
contractors must provide the City of Hot Springs with a valid IRS Taxpayer Identification Number (TIN), Unique Entity
ID number (UEI) for the physical business location responsible for managing the contract, and have an up-to-date
registration in the System for Award Management (SAM.gov) database demonstrating no suspensions, debarments
or other restrictions that would prevent a contract award.
Bid responses will be accepted from contractors without a TIN, UEI, and/or SAM record, but no award will be made
by the City of Hot Springs without timely receipt of this documentation upon request.
Respondents hereby certify that the information is accurate and complete to the best of their
knowledge, under penalty of law and verifiable by federal government representatives, and any
information not provided herein will be provided immediately upon request to the City of Hot Springs in
order to be considered eligible for contract award.
5 STATEMENT OF REQUIREMENTS
The following requirements from the Respondent will be shown below on "Certificates Required" and "Additional
Requirements."
6 Certificate Required
Certificates for coverages and items below shall be submitted by the Respondent with the proposal. If nothing listed
below, no insurance requirements apply.
7 General Liability (Minimum Amount) $500,000
Respondent is required to submit the certificate in the response attachments.
8 Business License
Respondent shall provide a copy of your company's/firm's business license certificate. The successful Respondent
may be required to obtain a City of Hot Springs business license. For business services within the City of Hot
Springs, the City will honor business licenses from cities within the State of Arkansas.
9 Certificate of Good Standing with the State's Secretary of State
Provide a copy of the Certificate of Good Standing with the Secretary of State's.
