1 INTRODUCTION
The City of Hot Springs, Arkansas, is seeking proposals for uniform service for Intracity Transit as a Term Contract
(TC). This Request for Proposals (RFP) contains all the information necessary to prepare and submit a proposal.
Prospective proposers are advised to study this RFP completely and follow the instructions exactly.

3 SCOPE OF SERVICE
The City of Hot Springs is requesting proposals to have a vendor provide uniform rental services and related
services for Intracity Transit at 100 Broadway Terrace as described herein as a Term Contract. The uniform laundry
service is to be provided weekly throughout the term of the contract. Seasonal changes will be determined by the
City/Department.
Uniforms are to be provided in a reasonably clean, good, and usable condition as determined by the City. Uniforms
deemed unusable by the City will be replaced by the Vendor at no additional cost to the City (this is in reference to
uniforms that are thin and worn because of the life of the unit due to normal wear and tear). The Vendor and the
City will establish a routine whereby each employee has available adequate, clean, usable uniforms at all times. The
Vendor is to come to the appropriate City facility to obtain individual measurements. Some, if not all, uniforms will
require employee names and the City logo or Department name. Additional sets for new employees, if needed, will
be at the 'unit' price as a proposal with no setup charges.
Billing is to be by monthly invoice. The deliverer of the uniforms must get a signature of a City Employee on the
contents of each delivery made. Each Department will designate those authorized to sign. The Vendor shall provide
pants and shirts for fourteen (14) employees and jackets for nineteen (19) employees. The following description for
uniforms at Intracity Transit:
Sizes range from Medium to XXX-Large
Gray cargo pants
Gray cargo shorts
Burgundy short-sleeve polo shirts with a pocket and Hot Springs logo
Burgundy short-sleeve casuals button front shirts with Hot Springs Logo
Gray jacket with removable liner with Hot Springs Logo
The City would like the Vendor to be available for an oral presentations by providing the City with
material samples.
The acquisition will use a grant from the Federal Transit Administration (FTA), and the FTA Third-Party clauses will
be included in the contract. The contract resulting from this RFP will be for one (1) year with four(4) twelve (12)
month terms, subject to annual budget approval by the Hot Springs Board of Directors. Contract will automatically
renew annually unless either party supplies thirty (30) day written Notice of Cancellation. Any escalation in price
shall be subject to paragraph 29 of the Terms and Conditions of Bidding. City reserves the right to accept or decline
any and all price increases.
4 FEDERAL AWARD IDENTIFICATION - SYSTEM FOR AWARD MANAGEMENT (SAM)
In order to be eligible for a contract award assisted by the Federal Transit Administration (FTA), all prime
contractors must be registered to do business with the U.S. Government.
Prior to any informal contract commitment and published notice of intent to award by the City of Hot Springs, eligible
contractors must provide the City of Hot Springs with a valid IRS Taxpayer Identification Number (TIN), Unique Entity
ID number (UEI) for the physical business location responsible for managing the contract, and have an up-to-date
registration in the System for Award Management (SAM.gov) database demonstrating no suspensions, debarments
or other restrictions that would prevent a contract award.
Respondents will be accepted from contractors without a TIN, UEI, and/or SAM record, but no award will be made by
the City of Hot Springs without timely receipt of this documentation upon request.
Respondents hereby certify that the information is accurate and complete to the best of their
knowledge, under penalty of law and verifiable by federal government representatives, and any
information not provided herein will be provided immediately upon request to the City of Hot Springs in
order to be considered eligible for contract award.
5 STATEMENT OF REQUIREMENTS
The following requirements from the Respondent will be shown below on "Certificates Required" and "Additional
Requirements."

6 Certificates Required
Certificates for coverages and items below must be submitted by the Respondent with the bid forms. If nothing listed
below, no insurance requirements apply.
7 General Liability (Minimum Amount) $500,000
Respondent is required to submit the certificate in the response attachments.
8 Business License
Respondent shall provide a copy of your company's/firm's business license certificate. The successful Respondent
may be required to obtain a City of Hot Springs business license. For business services within the City of Hot
Springs, the City will honor business licenses from cities within the State of Arkansas.
9 Certificate of Good Standing with the State's Secretary of State
Provide a copy of the Certificate of Good Standing with the Secretary of State's.

1 Sales Tax
3 Taxes - Taxable goods and services delivered inside the City of Hot Springs will have a tax rate of 9.5%. This sales
tax should be included in the bid price. All other goods and services will be taxed at the rate required by the
applicable governing bodies.
Respondent has included sales tax with the total price. If Respondent stated that they did not include sales tax, they
must explain why.
