Description:

The Napa County Health and Human Services Agency desires to solicit qualified proposals to provide Independent Living Program (ILP) services for youth in foster care or formerly in foster care.

The ILP program shall provide foster youth with greater self-sufficiency prior to leaving the foster care system by providing independent living skills assessments, training and case management services to assist in the transition to a successful, independent adulthood. Examples of skills that shall be taught are home management skills, such as locating appropriate housing, finding and maintaining employment, cooking, first aid, money management, substance abuse prevention and preventative health activities, such as smoking avoidance, nutrition education and pregnancy prevention. Youth shall be given information and assistance in applying and enrolling in post-secondary college education or vocational training programs. Youth shall also be given the opportunity to develop interpersonal/social skills such as communication, problem solving, time-management, building trust, networking and developing positive relationships. Youth shall be provided with personal and emotional support as needed prior to and upon aging out of the foster care and probation systems, through mentorship and the promotion of interactions with dedicated adults.

SERVICES
The Napa County Health and Human Services Agency desires to solicit qualified proposals to
provide Independent Living Program (ILP) services for youth in foster care or formerly in foster
care.
The ILP program shall provide foster youth with greater self-sufficiency prior to leaving the
foster care system by providing independent living skills assessments, training and case

CURRENT PRACTICES
Currently, Napa County Child Welfare offers Independent Living Program (ILP) services through
a contract with a non-profit agency located in Napa. The ILP program provides supportive
services to assist youth from Napa County and youth residing in Napa County to assist in areas
such as daily living skills, money management, finding and obtaining housing, vocational
training, education and job assistance. The ILP program serves youth in foster care or formerly
in foster care or probation between the ages of 15.5 through 21.
Referrals are submitted to the contractor by Children's Welfare Services (CWS) of Health and
Human Services Agency and the Napa County Juvenile Probation Department.
SCOPE OF WORK
The successful respondent will be expected to perform services according to the following scope of
work:
CONTRACTOR shall track ILP eligible youth, who receives services, in county and out of
county being provided with services, current or former youth in care or probation.
CONTRACTOR shall provide leadership development for ILP youth including employing
youth staff and ILP youth volunteers, and an ILP Youth Council who will have primary
responsibility for youth friendly program development, implementation, and evaluation.

CONTRACTOR shall track youth participation in the ILP Youth Council. How many youth
participate, what trainings and workshops are provided and what activities occur from the
Youth Council.
CONTRACTOR shall track and maintain youth attendance and engagement for trainings
and workshops through the provision of sign in and sign out logs.
CONTRACTOR will develop an individualized independent living plan designed to track
progress toward education, employment, wellness and housing goals. Program successes
will be tracked, as well as identifying areas to strengthen pending the data gathered.
CONTRACTOR shall provide outreach and recruitment through monthly calendar of
events and workshops by flyers and mixed media using texts/media/email, etc.
CONTRACTOR shall provide outreach to 100% of eligible youth to inform them of the ILP
program and services available and to encourage participation and provide an in-depth
assessment to 75% of all ILP eligible youth including intake documentation, provision of
the Ansel Casey Life Skills pre-and post- assessments.
CONTRACTOR shall provide comprehensive case management to all youth participating
in Extended Foster Care by connecting them to core ILP services.
CONTRACTOR shall provide target case management and information and referral
services.
CONTRACTOR shall provide assistance in enrollment for Cal Fresh, Medi-Cal and General
Assistance in addition to workshops focused on financial literacy.
CONTRACTOR shall assess academic status and personal goals, create an academic
success plan with all participating youth which includes a focus on high school graduation
and college preparation, working in collaboration with the Napa County Office of
Education (NCOE) Foster Youth Liaison, as needed.
CONTRACTOR shall provide youth with computer access to develop skills in computer
programs such as word processing, graphics, and the use of the Internet. CONTRACTOR
shall utilize project based activities for youth to develop computer skills.
CONTRACTOR shall collaborate with community partners (Napa Valley College,
Guardian Scholars, etc.) to provide participating youth with information and individual
counseling on: college options and vocational training programs, timelines for applying to
programs, applying for scholarships and/or financial aid, college life and retention.
CONTRACTOR shall focus efforts in working with ILP eligible youth on essential life skills
as defined by the California Department of Social Services: employment training, daily
living skills, survival skills, training in choices and consequences, interpersonal social skills
training, and training regarding education and computer/Internet skills.
CONTRACTOR shall also provide health and wellness services including trauma
education, physical health, and social and emotional skills and support.
CONTRACTOR shall provide mentoring services through 1:1 youth support and
workshops.
CONTRACTOR shall review youth’s individualized goals and service plans every six
months to determine the appropriateness and relevance of the plan and to create/provide
update based on changing needs of each youth, including referring youth to other VOICES
programs and other co-located services housed at VOICES.
