The Department of Public Health & Environment (DDPHE), is seeking proposals from community partners that offer programs/services that align with the objectives of the Centers for Disease Control & Prevention (CDC) Public Health Infrastructure Grant.

Background

DDPHE was awarded a 5-year grant from the Centers for Disease Control and Prevention (CDC) for Strengthening U.S. Public Health Infrastructure, Workforce, and Data Systems (also known as the Public Health Infrastructure Grant (PHIG)). The intent of the PHIG funding is to: 1) improve public health outcomes by strengthening the capacity and systems related to the public health workforce and infrastructure in Denver communities, 2) strengthen and sustain community partnership development and engagement, and 3) direct resources in a way that supports diversity and health equity.

Supporting Community Partners who make up Denver’s Public Health Infrastructure Program Funding

As part of the Request for Proposal (RFP), DDPHE estimates the total awarded funding to be approximately $842,000 to community-based organizations that serve Denver communities. Organizations may submit one grant application requesting up to $75,000 for the contract term. The contract term is anticipated to begin in March/April 2024 and end November 30, 2024 with potential renewal funding through an annual renewal process each year from December 1, 2024 through November 30, 2027, contingent upon federal allocation and each awardees successful completion of year one of the project with compliance to the terms of the contract. Unobligated funds may be carried over into the next term per expanded authority granted by the CDC. Subrecipients will be notified no later than December 15th of each year of their carryover amount.

Organization cannot be de-barred and must have (or be able to have) a certificate of non-debarment, active registration on SAM.gov, and federal UEI #
Organization must have at least 12 months of operation and hold an active nonprofit status 501(c)3 at the time of application. This includes those organizations with a fiscal sponsor incorporated as a 501(c)3 and registered in the State of Colorado.
Organization must be in the City and County of Denver and/or primarily serve Denver residents with awarded funds.

Organization must be a community-based organization working at the local level to provide services to communities that are economically and socially marginalized.

Scope of Work

All proposed program/service activities must align with the objectives of the Centers for Disease Control & Prevention (CDC) Public Health Infrastructure Grant. Key activities are broad by design and intended to provide recipients with maximum flexibility to meet their needs.

Sample activities to consider include:

Improve collaborative systems and processes with behavioral health partners or provide behavioral health services in underserved communities.
Engage with communities to identify, prioritize, and address key health needs and gaps through collaborative processes.
Engage with partners in the health care system to improve health service availability and access to care.
Develop and implement multisector or system strategies to increase access to social services.
Build relationships with educational programs that promote the development of future public health workers, including partnerships to create pathways into the public health workforce.
Connect communities to services that support the whole person by providing robust linkages and navigation for community resources.

Required activities include:

Identify community health needs, inform public health intervention programs, and assist with public health workforce recruitment strategies for public health across Denver by:
- Participating in DDPHE’s Community Health Improvement Plan and Community Health Assessment.
- Participating in DDPHE’s Health Equity Community Advisory Group (CAG).
Develop a language access plan and protocol to communicate with and provide services to individuals with limited English proficiency. In compliance with Title VI of the Civil Rights Act of 1964, 2 C.S. § 561 et seq. (Act 172 of 2006), and City and County of Denver’s Executive Order NO. 150: Citywide Language Access Program
Participate in the evaluation of their project with the technical assistance support of a DDPHE evaluator.

Awards will be made to the applicants whose proposal meets the requirements of the RFP and is determined to be most

responsive, responsible, and best value to the City and County of Denver, in accordance with the scope of work,

community needs, and Denver Revised Municipal Code.

There is the potential for renewal funding to be awarded to each recipient annually through November 30, 2027.

However, this will be contingent upon future federal funding allocations to DDPHE’s Public Health Infrastructure Grant,

and each awardee’s successful completion of year one of the project with compliance to the terms of the contract.

Contracts may be extended via renewal funding annually in December 2024, 2025, and 2026.

B. ELIGIBILITY

Community-based Organization Eligibility Requirements:

• Organization must be (or be able to) register with the Colorado Secretary of State

• Organization cannot be de-barred and must have (or be able to have) a certificate of non-debarment, active

funds.

Entities that are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded

from participation in this transaction by any federal department or agency are not eligible.

Response reviews and awards are contingent upon respondents being current with the city on any loan, contractual, or

tax obligation that is due, and in compliance with rules, regulations, and provisions of existing or past city contracts.

C. SCOPE OF WORK

All proposed program/service activities must align with the objectives of the Centers for Disease Control & Prevention

(CDC) Public Health Infrastructure Grant. Key activities are broad by design and intended to provide recipients with

maximum flexibility to meet their needs. Sample activities to consider include:

• Improve collaborative systems and processes with behavioral health partners or provide behavioral health

Additional SOW Requirements for organizations that receive funding. Additional guidance can be found on page 10

and DDPHE will provide more information during contract execution.

• Identify community health needs, inform public health intervention programs, and assist with public health

workforce recruitment strategies for public health across Denver by:

o Participating in DDPHE’s Community Health Improvement Plan and Community Health Assessment.

o Participating in DDPHE’s Health Equity Community Advisory Group (CAG).

• Develop a language access plan and protocol to communicate with and provide services to individuals with

limited English proficiency. In compliance with Title VI of the Civil Rights Act of 1964, 2 C.S. § 561 et seq. (Act 172

of 2006), and City and County of Denver’s Executive Order NO. 150: Citywide Language Access Program

Applications must show strong fiscal responsibility with costs that are reasonable, realistic, and justified. Funds must be

used to supplement (not supplant, replace, or redirect) Federal, State, and/or local funding currently provided to the

organization for operating its program(s). The applicant agrees to comply with all Federal, State, and local laws and

regulations applicable to the funding source authorizing any program or activity funded through this RFP and with all

laws, policies, procedures, ordinances, and regulations of the City and County of Denver.

Each applicant’s proposal shall include a budget that reflects the operational needs of the program using the budget

spreadsheet template included in the RFP documents. Funds can only be used for the purposes outlined in the service

agreement.

Allowable program costs for this request for proposal

Funds can be used for a variety of purposes in the following categories: A) Personnel: staffing support, B) Fringe:

benefits for personnel as a percentage of personnel salaries listed in category A, C) Travel: reasonable transportation

costs for professional development, training, or outreach, D) Supplies & Materials: reasonable expenses for program

supplies and materials with appropriate justification, E) Other Direct Costs: other direct program costs that are not

covered in other categories, F) Indirect Costs: 10% of the amount requested is allowed for indirect/administrative costs.

Disallowed program costs for this request for proposal

Funds cannot be used for the purchase of equipment over $5,000, clinical care, medical supplies, medicine, vaccines,

food, alcoholic beverages, entertainment, fundraising or underwriting, marketing costs, research, legislative policy and

advocacy, political activities or partisan causes, endowments, loans or debt reduction, scholarships, reimbursement of

pre-award costs, lobbying, advertising, or propaganda.

Funds cannot be used to make large capital purchases such as the purchase of land or buildings.

Funds cannot be used to support religious practices, such as religious instruction, worship, or prayer. Faith-based

organizations may offer such practices, but at a separate time and location as the program applying for funding.
